摘要
目的探究标准化代谢性疾病管理中心(MMC)目标管理在糖尿病合并脑卒中患者护理中的应用效果。方法选取2019年1月至2021年8月江西省宜春市人民医院收治的80例脑卒中合并2型糖尿病患者作为研究对象,按照密封信封抽签方法将其分为对照组(40例)与观察组(40例),对照组采用血糖常规护理干预,观察组在对照组基础上利用MMC的配套设施实施血糖目标管理。比较两组患者的干预前和干预6个月后的空腹血糖、餐后2h血糖、糖化血红蛋白、体重指数(BMI)和自我管理行为评分、生活能力(ADL)评分。结果两组患者干预前空腹血糖、餐后2 h血糖、糖化血红蛋白水平、BMI、腰围及腹围比较,差异无统计学意义(P>0.05)。干预6个月后,两组患者空腹血糖、餐后2 h血糖、糖化血红蛋白水平及BMI均低于本组干预前,腰围、腹围均短于本组干预前,差异有统计学意义(P<0.05)。干预6个月后,观察者患者的空腹血糖、餐后2 h血糖、糖化血红蛋白水平均低于对照组,差异有统计学意义(P<0.05)。干预6个月后,观察组患者的BMI低于对照组,观察组患者的腰围、腹围均短于对照组,差异有统计学意义(P<0.05)。两组患者干预前糖尿病自我管理行为量表(SDSCA)、生活能力(ADL)评分比较,差异无统计学意义(P>0.05)。干预6个月后,两组患者的SDSCA、ADL评分均高于本组干预前,差异有统计学意义(P<0.05)。干预6个月后,观察组患者的SDSCA、ADL评分高于对照组,差异有统计学意义(P<0.05)。结论MMC目标管理在糖尿病合并脑卒中患者护理中的应用,能够有效控制血糖,降低体重,提高患者自理能力及生活质量,值得推广。
Objective To explore the application effect of Metabolic Management Center(MMC)target management in the nursing of patients with diabetes complicated with stroke.Methods A total of 80 patients with stroke complicated with type 2 diabetes admitted to Yichun People's Hospital of Jiangxi Province from January 2019 to August 2021 were selected as research objects.According to the sealed envelope lottery method,they were divided into control group(40 cases)and observation group(40 cases).The control group received routine nursing intervention of blood glucose.On the basis of the control group,the blood glucose target management was implemented in the observation group using the supporting facilities of MMC.Fasting blood glucose,2 h postprandial blood glucose,glycosylated hemoglobin,body mass index(BMI),self-management behavior score and activity of daily living(ADL)score were compared between the two groups before intervention and 6 months after intervention.Results There were no statistically significant differences between the two groups in fasting blood glucose,2 h postprandial blood glucose,glycosylated hemoglobin level,BMI,waist circumference and abdominal circumference(P>0.05).After 6 months of intervention,the fasting blood glucose,2 h postprandial blood glucose,glycosylated hemoglobin level and BMI of the two groups were lower than those before the intervention,and the waist circumference and abdominal circumference of the two groups were shorter than those before the intervention,the differences were statistically significant(P<0.05).After 6 months of intervention,the fasting blood glucose,2 h postprandial blood glucose and glycosylated hemoglobin levels of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).After 6 months of intervention,the BMI of the patients in the observation group was lower than that in the control group,and the waist circumference and abdominal circumference of the patients in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).There were no significant differences in the scores of summary of diabetes self-care activities(SDSCA)and activity of daily living(ADL)between the two groups before intervention(P>0.05).After 6 months of intervention,the SDSCA and ADL scores of the two groups were higher than those of the group before intervention,and the differences were statistically significant(P<0.05).After 6 months of intervention,the SDSCA and ADL scores of patients in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion The application of MMC objective management in the nursing of patients with diabetes complicated with stroke can effectively control blood glucose,reduce body weight,improve patients'self-care ability and quality of life,which is worth promoting.
