摘要
目的 评价参芪温阳阴道膨胀栓与盆底生物反馈电刺激治疗Ⅰ、Ⅱ度盆腔器官脱垂(POP)的有效性与安全性。方法 240例Ⅰ、Ⅱ度POP患者,采用随机对照法分为试验组和对照组,每组120例。试验组患者接受参芪温阳阴道膨胀栓治疗,对照组患者接受生物反馈电刺激治疗。比较两组患者的治疗效果、中医证候积分、盆底肌电评分、盆腔器官脱垂定量分度法(POP-Q)分度情况、治疗依从性、不良反应/不良事件发生情况。结果 治疗4、8周后,两组患者中医证候积分均显著低于本组治疗前,且试验组患者中医证候积分(5.90±0.29)、(2.37±0.78)分低于对照组的(9.98±0.39)、(5.23±0.90)分,差异具有统计学意义(P<0.05)。试验组患者治疗总有效率为80.00%,明显高于对照组的60.83%,差异有统计学意义(P<0.05)。治疗4、8周后,两组患者盆底肌电评分均高于本组治疗前,且试验组患者盆底肌电评分(82.81±11.79)、(91.78±12.89)分高于对照组的(79.47±13.97)、(85.66±12.58)分,差异具有统计学意义(P<0.05)。治疗8周后,试验组患者POP-Q分度优于对照组,差异具有统计学意义(P<0.05)。试验组患者总依从率为83.33%,高于对照组的67.50%,差异具有统计学意义(P<0.05)。试验组患者不良反应/不良事件发生率为16.67%,低于对照组的32.50%,差异具有统计学意义(P<0.05)。结论 与生物反馈电刺激比较,参芪温阳阴道膨胀栓治疗Ⅰ、Ⅱ度POP的临床效果显著,中医证候、盆底肌电得到明显改善,且治疗依从性、安全性高,值得临床推广应用。
Objective To evaluate the efficacy and safety of Shenqi Wenyang vaginal dilation suppository and biofeedback and pelvic floor electrical stimulation in the treatment of stageⅠandⅡpelvic organ prolapse(POP).Methods A total of 240 patients with stageⅠandⅡPOP were randomly divided into experimental group and control group,with 120 cases in each group.The experimental group was treated with Shenqi Wenyang vaginal dilation suppository,and the control group was treated with biofeedback and electrical stimulation.Both groups were compared in terms of therapeutic effect,traditional Chinese medicine syndrome score,score of pelvic floor electromyography,pelvic organ prolapse-quantification system(POP-Q),treatment compliance,adverse reactions/adverse events.Results After 4 and 8 weeks of treatment,the traditional Chinese medicine syndrome scores in both groups were significantly lower than those before treatment in this group;the traditional Chinese medicine syndrome scores were(5.90±0.29)and(2.37±0.78)points in the experimental group,which were lower than those of(9.98±0.39)and(5.23±0.90)points in the control group;the differences were statistically significant(P<0.05).The total effective rate of 80.00%in the experimental group was significantly higher than that of 60.83%in the control group,and the difference was statistically significant(P<0.05).After 4 and 8 weeks of treatment,the scores of pelvic floor electromyography in both groups were higher than those before treatment in this group;the scores of pelvic floor electromyography were(82.81±11.79)and(91.78±12.89)points in the experimental group,which were higher than those of(79.47±13.97)and(85.66±12.58)points in the control group;the differences were statistically significant(P<0.05).After 8 weeks of treatment,POP-Q score of the experimental group was better than that of the control group,and the difference was statistically significant(P<0.05).The total compliance rate of 83.33%in the experimental group was higher than that of 67.50%in the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions/adverse events in the experimental group was 16.67%,which was lower than that of 32.50%in the control group,and the difference was statistically significant(P<0.05).Conclusion Compared with biofeedback and electrical stimulation,Shenqi Wenyang vaginal dilation suppository has significant clinical effect in the treatment of stageⅠandⅡPOP,which can significantly improve the traditional Chinese medicine syndrome and pelvic floor electromyography,and the treatment compliance and safety are high.It is worthy of clinical promotion and application.
