摘要
目的探究β-人促绒毛膜促性腺激素(β-hCG)与孕酮(P)在早孕、早期输卵管妊娠和早期先兆流产诊断中的检测价值。方法以2020年1月-2022年6月湖口县妇幼保健院接收的100例早孕女性为研究对象,其中正常早孕女性34例、早期输卵管妊娠女性33例、早期先兆流产女性33例,另取同期体检的30名未孕女性作为对照,比较以上研究对象的血清β-hCG、P水平,分析β-hCG、P及联合检测对早孕、早期输卵管妊娠、早期先兆流产的诊断价值。结果β-hCG(首次、48 h)与P水平由高至低依次为正常早孕女性>早期先兆流产女性>早期输卵管妊娠女性>未孕女性(P<0.05),且正常早孕女性与早期先兆流产女性48 hβ-hCG水平高于首次检测(P<0.05),而早期输卵管妊娠女性48 hβ-hCG水平与首次检测比较,差异无统计学意义(P>0.05);β-hCG检测与P检测对早孕、早期输卵管妊娠、早期先兆流产的诊断准确性比较,差异无统计学意义(P>0.05),β-hCG与P联合检测对早孕、早期输卵管妊娠、早期先兆流产的诊断准确性高于二者单独检测(P<0.05)。结论β-hCG与P检测对早孕、早期输卵管妊娠、早期先兆流产的诊断具有指导作用,二者联合检测具有更高的诊断准确性。
Objective To explore the detection value ofβ-human chorionic gonadotropin(β-hCG)and progesterone(P)in the diagnosis of early pregnancy,early tubal pregnancy and early threatened abortion.Methods From January 2020 to June 2022,100 women with early pregnancy received by Hukou County Maternal and Child Health Hospital were selected as the research objects,including 34 women with normal early pregnancy,33 women with early tubal pregnancy and 33 women with early threatened abortion.In addition,30 non-pregnant women who underwent physical examination during the same period were selected as the control group.The serumβ-hCG and P levels of the above subjects were compared,and the diagnostic values ofβ-hCG,P and combined detection for early pregnancy,early tubal pregnancy and early threatened abortion were analyzed.Results The levels ofβ-hCG(the first time,48 h)and P from high to low were normal early pregnancy women>early threatened abortion women>early tubal pregnancy women>non-pregnant women(P<0.05),and the levels ofβ-hCG at 48 h in normal early pregnancy women and early threatened abortion women were higher than those in the first test(P<0.05),while the levels ofβ-hCG at 48 h in early tubal pregnancy women were not significantly different from those in the first test(P>0.05).There was no significant difference in the diagnostic accuracy ofβ-hCG detection and P detection for early pregnancy,early tubal pregnancy and early threatened abortion(P>0.05).The diagnostic accuracy of combined detection ofβ-hCG and P for early pregnancy,early tubal pregnancy and early threatened abortion was higher than that of single detection(P<0.05).Conclusion The detection ofβ-hCG and P has a guiding role in the diagnosis of early pregnancy,early tubal pregnancy and early threatened abortion.The combined detection of the two has higher diagnostic accuracy.
