多重耐药菌耐药性及抗菌药物的临床合理使用分析

摘要 目的分析多重耐药菌(MDROS)的耐药性和抗菌药物使用情况,以预防MDROS感染。方法选取2019年1月至2020年1月于本院治疗的100例多重病原菌患者作为研究对象,进行菌株鉴定,分析检出MDROS菌株种类,并进行药敏试验统计耐药率及抗菌药物使用情况。结果100例患者共检出145株MDROS,其中大肠埃希菌(E coli)占比最高,为41.38%;其次为金黄色葡萄球菌(SA),占比29.66%;之后依次为肺炎克雷伯菌(Kpn)占比、鲍氏不动杆菌(Aba)、铜绿假单胞菌(PAE)、其他,占比分别为15.17%、6.90%、4.83%、2.07%。E coli和Kpn对氨苄西林、头孢唑啉的耐药率均为100.00%,对亚胺培南的耐药率均为0.00%。SA对青霉素G、苯唑西林的耐药率均为100.00%,对万古霉素等耐药率为0.00%。不合理用药占比58.00%,主要包括未按照感染情况使用多种药物、未根据药物规定溶媒、给药次数过多或过少、用药疗程不足或超长、用药剂量不正确及无指征用药。结论本院多重耐药菌抗菌药物使用不合理情况较严重,临床医师应加强抗菌药物的合理使用,采用相应干预措施降低MDROS感染率,保证医疗质量。 Objective To analyze the resistance and antimicrobial use of multidrug-resistant organism(MDROS)to prevent MDROS infection.Methods A total of 100 patients with MDROS treated in our hospital from January 2019 to January 2020 were selected as research subjects for strain identification,MDROS strain species were analyzed,and drug susceptibility tests were performed to calculate the resistance rate and antibacterial use of antimicrobial drugs.Results A total of 145 strains of MDROS were detected in 100 patients,among which Escherichia coli(E coli)accounted for the highest proportion(41.38%);followed by Staphylococcus aureus(SA)accounted for 29.66%,followed by Klebsiella pneumoniae(Kpn),acinetobacter bouruginosis(Aba),pseudomonas aeruginosa(PAE)and others accounted for 15.17%,6.90%,4.83%,2.07%,respectively.The resistance rates of E coli and Kpn to ampicillin and cefazoline were 100.00%.The resistance rate to imipenem was 0.00%.The resistance rate of SA to penicillin G and oxacillin was 100.00%,and the resistance rate to vancomycin was 0.00%.Irrational drug use accounting for 58.00%,and unreasonable situations mainly included the use of multiple drugs without following the infection situation according to the drug,too many or too few dosing,insufficient or long course of medication,incorrect dosage and unindicated medication.Conclusion The unreasonable use of antibacterial drugs by multidrug-resistant bacteria in our hospital is serious,and clinicians should strengthen the rational use of antibacterial drugs,and adopt corresponding interventions to reduce MDROS infection rate and ensure medical quality.
