经鼻高流量湿化氧疗对支气管扩张症合并感染患者HIF-1α、IL-17及TGF-β1的影响 被引量:3

摘要 目的分析经鼻高流量湿化氧疗对支气管扩张症合并感染患者缺氧诱导因子⁃1α(HIF⁃1α)、白介素⁃17(IL⁃17)及转化生长因子⁃β1(TGF⁃β1)的影响。方法选择2020年5月至2022年5月河北省老年病医院收治的129例支气管扩张症合并感染患者,根据不同治疗方式将其分为对照组(63例,经鼻持续正压通气疗法治疗)和观察组(66例,经鼻高流量鼻导管湿化氧疗治疗),观察两组治疗前、治疗2周后的动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))、氧合指数(PaO_(2)/FiO_(2))、血清HIF⁃1α、IL⁃17及TGF⁃β1水平、临床疗效及并发症。结果观察组PaO_(2)、PaO_(2)/FiO_(2)高于对照组,PaCO_(2)低于对照组,差异有统计学意义(P<0.05);观察组HIF⁃1α、IL⁃17、TGF⁃β1水平低于对照组,差异有统计学意义(P<0.05);观察组治疗总有疗效为93.94%,明显高于对照组的80.95%,差异有统计学意义(P<0.05);观察组并发症发生率为9.10%,明显低于对照组的23.82%,差异有统计学意义(P<0.05)。结论经鼻高流量湿化氧疗治疗能调节支气管扩张症合并感染患者HIF⁃1α、IL⁃17及TGF⁃β1水平,可有效改善患者发缺氧症状,具有较高的临床疗效和安全性。 Objective To analyze the effect of nasal high flow humidification oxygen therapy on hypoxia inducible factor⁃1α(HIF⁃1α),interleukin⁃17(IL⁃17)and transforming growth factor⁃β1(TGF⁃β1)in patients with bronchiectasis complicated with infection.Methods 129 patients with bronchiectasis and infection admitted to Hebei Geriatric Hospital from May 2020 to May 2022 were selected,and according to the different treatment methods,they were divided into the control group(63 cases,treated by nasal continuous positive pressure ventilation)and the observation group(66 cases,treated by nasal high⁃flow nasal cannula humidification and oxygen therapy),the Arterial partial pressure of oxygen(PaO_(2)),arterial partial pressure of carbon dioxide(PaCO_(2)),oxygenation index(PaO_(2)/FiO_(2)),serum HIF⁃1α,IL⁃17 and TGF⁃β1 levels,clinical efficacy and complications before and after 2 weeks of treatment of the two groups were observed.Results The PaO_(2)and PaO_(2)/FiO_(2)in the observation group were higher than those in the control group,while PaCO_(2)in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).The levels of HIF⁃1α,IL⁃17 and TGF⁃β1 in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The total effective rate of treatment in the observation group was 93.94%,which was significantly higher than 80.95%in the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the observation group was 9.10%,which was significantly lower than that in the control group(23.82%),and the difference was statistically significant(P<0.05).Conclusion Nasal high⁃flow humidified oxygen therapy can regulate the levels of HIF⁃1α,IL⁃17 and TGF⁃β1 in patients with bronchiectasis complicated by infection,and can effectively improve the symptoms of hypoxia,with high clinical efficacy and safety.
