摘要
目的分析青少年特发性脊柱侧弯症(adolescent idiopathic scoliosis,AIS)患者的步态参数特征,探讨AIS患者在静止站立状态下的负重平衡性和动态行走状态下的步态运动学变化。方法选取2021年5月至2021年9月山西医科大学第二医院脊柱病区首次就诊的10~18岁的胸腰段AIS患者30例,以脊柱侧弯方向的不同分为三组。左侧弯组9例,男4例,女5例;年龄11~15岁,平均(13.33±2.45)岁;右侧弯组10例,男3例,女7例;年龄12~14岁,平均(13.30±2.00)岁;“S”型侧弯组11例,男3例,女8例;年龄11~15岁,平均(13.18±1.47)岁。另招募10~18岁10例健康青少年为对照组,男4例,女6例;年龄8~13岁,平均(11.50±1.27)岁。利用三维步态分析仪采集四组受试者静态站立及行走状态下的时空参数、关节运动学参数等指标进行检测和比较,以分析不同弯曲方向的AIS患者负重情况与步态特征。结果(1)负重情况方面:四组AIS患者静态站立和动态行走时的负重平衡性比较差异有统计学意义(P<0.05);(2)步态参数。①时空参数:四组受试者步长比较差异有统计学意义(P<0.05);②运动学参数:右侧弯组两侧髋、膝、踝关节活动范围比较差异有统计学意义(P<0.05)。结论脊柱侧弯会改变全身的姿势控制和步态模式。不同侧弯方向AIS患者的静态站立负重、平衡性、步态分析时空和运动学参数差异有统计学意义。未来需进一步深入研究,以寻求AIS患者更多步态参数规律。
Objective Objective:To analyze the gait parameters of adolescent idiopathic scoliosis(AIS)patients,and to explore the changes of weight-bearing balance and gait kinematics in dynamic walking in patients with AIS.Methods A total of 30 patients with thoracolumbar AIS aged 10~18 who visited the Spine Ward of the Second Hospital of Shanxi Medical University for the first time from May 2021 to September 2021 were enrolled.The patients were divided into three groups according to the direction of scoliosis.There were 9 cases in the left curve group,inlcuding 4 males and 5 females.The age was 11~15 years old,with an average ago of(13.33±2.45)years.Right curve group had 10 cases,including 3 males and 7 females.The age was 12~14 years old,with an average ago of(13.30±2.00)years.There were 11 cases of thoracolumbar scoliosis of the“S”spine,3 males and 8 females.The age was 11~15 years old,and the average age was(13.18±1.47)years.In addition,10 healthy adolescents aged 10~18 years were recruited as the control group,including 4 males and 6 females.The age was 8~13 years old,with an average age of(11.50±1.27)years.The 3D gait analyzer was used to collect the spatiotemporal parameters,joint kinematic parameters and other indicators in the static standing and walking states of the four groups of subjects for detection and comparison,so as to analyze the weight-bearing and gait characteristics of AIS patients in different bending directions.Results(1)Weight bearing status:There were significant differences in weight balance between static standing and dynamic walking between the four groups of AIS patients(P<0.05)and.(2)Gait parameters.①Spatial-temporal parameters:The difference in step length between the four groups was statistically significant(P<0.05).②Kinematic parameters:There were significant differences in the range of motion of hip,knee and ankle joints on both sides of the right curve group(P<0.05).Conclusion Scoliosis alters posture control and gait patterns throughout the body.There are significant differences in static standing weight-bearing and balance,gait analysis temporal and spatiotemporal and kinematic parameters of AIS patients with different lateral bending directions.Further research is needed in the future to seek more gait parameter patterns in AIS patients.
