摘要
目的分析彩色多普勒超声诊断老年退行性心脏瓣膜病(SDHVD)的价值。方法回顾性分析2018年3月至2020年11月南阳市第一人民医院收治的134例疑似SDHVD患者的临床资料,均行彩色多普勒超声诊断,以临床综合诊断结果为"金标准",统计彩色多普勒超声的诊断效能、检出率。结果134例疑似SDHVD患者经临床综合诊断结果证实阳性107例,阴性27例;采用彩色多普勒超声诊断证实阳性100例,阴性22例。彩色多普勒超声诊断SDHVD的灵敏度、特异度、准确度、漏诊率、误诊率、阳性预测值、阴性预测、约登指数分别为93.46%(100/107)、81.48%(22/27)、91.04%(122/134)、6.54%(7/107)、18.52%(5/27)、95.24%(100/105)、75.86%(22/29)、0.75。在临床综合诊断结果证实的107例SDHVD患者中,不同年龄段均具有较高的检出率,但患者多为主动脉瓣病变,二尖瓣和联合瓣膜病变者相对较少。结论彩色多普勒超声对SDHVD患者具有较好的诊断效能,阳性检出情况和不同年龄、不同类型检出情况良好,能满足患者和临床医师诊断需求,可为临床诊疗提供科学参考。
Objective To analyze the application value of color Doppler ultrasound in diagnosing senile degenerative heart valvular disease(SDHVD).Methods The clinical data of 134 patients with suspected SDHVD treated in Nanyang First People’s Hospital from March 2018 to November 2020 were retrospectively analyzed,and all of them underwent color Doppler ultrasound examination.Taking the comprehensive clinical diagnosis results as the gold standard,the diagnostic efficacy and detection rate of color Doppler ultrasound were counted.Results Among the 134 suspected SDHVD patients,107 cases were positive,and 27 cases were negative according to clinical comprehensive diagnosis;100 cases were positive,and 22 cases were negative according to color Doppler ultrasound diagnosis.The sensitivity,specificity,accuracy,missed diagnosis rate,misdiagnosis rate,positive predictive value,negative predictive value and Jordan index of color Doppler ultrasound in the diagnosis of SDHVD were 93.46%(100/107),81.48%(22/27),91.04%(122/134),6.54%(7/107),18.52%(5/27),95.24%(100/105),75.86%(22/29)and 0.75,respectively.Among the 107 patients with SDHVD confirmed by the comprehensive clinical diagnosis results,the detection rate of patients from different age groups all were high,but most of the patients had aortic valve diseases,and relatively few had mitral valve and valvular disease.Conclusions Color Doppler ultrasound has a good diagnostic effect in the diagnosis of SDHVD.The positive detection and detection of patients at different ages and types are good.It can meet the diagnostic needs of patients and clinicians,and can provide scientific reference for clinical diagnosis and treatment.
