摘要
[目的]总结国医大师卢芳运用加味泻黄散治疗心脾积热型白塞病的临床经验。[方法]分析卢老对心脾积热型白塞病病因病机的认识,剖析加味泻黄散组成、方义,并举临床案例加以验证。[结果]卢老认为心脾积热型白塞病的病因病机为荣卫失和,湿热壅滞心脾,毒邪蕴于肌肤则发溃疡蚀烂。以清胃泻火、解毒疗疮为治则,并运用加味泻黄散治疗此病,临证时又可根据溃疡部位选取不同药物,从理法方药上论证了加味泻黄散治疗心脾积热型白塞病的合理性和有效性。所举验案辨为心脾积热证,予加味泻黄散合三物黄芩汤治疗正符合其病机,患者症状得到显著改善。[结论]卢老运用加味泻黄散治疗心脾积热型白塞病疗效显著,也是对古方泻黄散的传承与创新,值得临床学习。
[Objective]To summarize the clinical experience of Chinese Medicine Great Master LU Fang in the treatment of Behcet's disease of accumulated heat in heart and spleen type with Jiawei Xiehuang Powder.[Methods]This paper analyzed Professor LU Fang's understanding of the etiology and pathogenesis of Behcet's disease of accumulated heat in heart and spleen type,analyzed the composition and the meaning of Jiawei Xiehuang Powder,and cited a clinical case to verify.[Results]Professor LU Fang believes that the etiology and pathogenesis of Behcet's disease of accumulated heat in heart and spleen type is due to the disharmony of nutrient and defense,dampness and heat obstructing the heart and spleen,and toxic evil accumulated in the skin,then it causes ulceration and erosion,the treatment principle is to clear the stomach and purify fire,detoxify and treat sores,and Jiawei Xiehuang Powder is used to treat the disease,different drugs can be selected according to the ulcer site in clinic,which verifies the rationality and effectiveness of Jiawei Xiehuang Powder in the treatment of Behcet's disease of accumulated heat in heart and spleen type from the perspective of principle,method,recipe and medicines.The case mentioned was Behcet's disease of accumulated heat in heart and spleen type and treated with Jiawei Xiehuang Powder and Sanwu Huangqin Decoction,which was in line with its pathogenesis,and the symptoms were significantly relieved.[Conclusion]Professor LU Fang's application of Jiawei Xiehuang Powder in the treatment of Behcet's disease of accumulated heat in heart and spleen type has a significant effect,which is also the inheritance and innovation of the ancient prescription Xiehuang Powder,and worthy of clinical learning.
