摘要
目的:探讨保乳手术与改良根治术在早期乳腺癌治疗中的应用效果。方法:选取2020年1月—2022年1月厦门大学附属第一医院收治的76例早期乳腺癌患者,按随机数表法分为两组,各38例。对照组予以改良根治术治疗,观察组予以保乳手术治疗,随访6个月。比较两组手术情况、应激反应、肩关节活动度、生活质量及并发症。结果:观察组手术时间、住院时间短于对照组,术中出血量、术后引流量少于对照组,差异有统计学意义(P<0.05);观察组术后1 d皮质醇(Cor)、去甲肾上腺素(NE)、促肾上腺皮质激素(ACTH)水平低于对照组,差异有统计学意义(P<0.05);观察组术后1个月肩关节外旋、内旋及屈曲活动度高于对照组,差异有统计学意义(P<0.05);观察组术后6个月心理、生理、社会及环境领域评分高于对照组,并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:保乳手术治疗早期乳腺癌效果更佳,能减轻手术创伤,减小对肩关节活动度的影响,且并发症少,利于改善患者生活质量。
Objective:To investigate the effect of breast conserving surgery and modified radical mastectomy in the treatment of early breast cancer.Method:A total of 76 patients with early breast cancer admitted to the First Affiliated Hospital of Xiamen University from January 2020 to January 2022 were selected,and divided into two groups according to the random number table method,with 38 cases in each group.The control group was treated with modified radical operation,and the observation group was treated with breast conserving operation,with a follow-up of 6 months.The operation condition,stress reaction,shoulder joint range of motion,quality of life and complications were compared between the two groups.Result:The operation time and hospital stay in the observation group were shorter than those in the control group,and the intraoperative bleeding volume and postoperative drainage volume in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).One day after operation,the levels of cortisol(Cor),norepinephrine(NE)and adrenocorticotropic hormone(ACTH)in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).One month after operation,the degree of lateral rotation,internal rotation and flexion of the shoulder joint in the observation group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).The scores of psychological,physiological,social and environmental fields in the observation group 6 months after operation were higher than those in the control group,and incidence of the complications were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Breast conserving operation is more effective in the treatment of early breast cancer.It can reduce the surgical trauma,reduce the impact on shoulder joint mobility,and has fewer complications,which is conducive to improving the quality of life of patients.
