多模态超声对浅表结核性淋巴结炎的诊断价值 被引量:2

Diagnostic Value of Multimodal Ultrasound in Superficial Tuberculous Lymphadenitis

摘要 目的:探讨多模态超声在鉴别浅表结核性淋巴结炎(TBL)中的诊断价值。方法:回顾性分析2017年12月-2021年12月鹰潭市余江区中医院连续收治的90例浅表淋巴结肿大患者的临床资料,经临床病理或实验室检查确诊为TBL 51例(观察组),非TBL 39例(对照组)。比较两组彩色多普勒超声、超声弹性成像、对比增强超声临床特征差异,以病理检测结果为对照,计算彩色多普勒超声、超声弹性成像、对比增强超声及多模态超声鉴别浅表TBL的敏感度、特异度、阳性预测值、阴性预测值及受试者工作特征(ROC)曲线下面积(AUC)。结果:两组患者彩色多普勒超声、超声弹性成像和对比增强超声特征比较,差异均有统计学意义(P<0.05)。彩色多普勒、超声弹性成像联合多模态超声诊断AUC为0.861,彩色多普勒、超声弹性成像和对比增强超声联合多模态超声的AUC为0.935,均高于彩色多普勒超声、超声弹性成像和对比增强超声诊断的AUC(P<0.05)。彩色多普勒检查的敏感度、特异度和准确性分别为80.39%、66.67%和74.44%;超声弹性成像的敏感度、特异度和准确性分别为76.47%、64.10%和71.11%;对比增强超声的敏感度、特异度和准确性分别为84.31%、61.54%和74.44%;彩色多普勒、超声弹性成像联合多模态超声的敏感度、特异度和准确性分别为90.20%、82.05%和86.67%;彩色多普勒超声、超声弹性成像和对比增强超声联合多模态超声的敏感度、特异度和准确性分别为92.16%、94.87%和93.33%。结论:多模态超声对浅表TBL的鉴别诊断具有较高的应用价值。 Objective:To investigate the diagnostic value of multimodal ultrasound in differential diagnosis of superficial tuberculous lymphadenitis(TBL).Method:The clinical data of 90 patients with superficial lymphadenopathy admitted to Yujiang District Hospital of Traditional Chinese Medicine,Yingtan City from December 2017 to December 2021 were retrospectively analyzed.51 patients(observation group)were diagnosed as TBL by clinical pathology or laboratory examination,and 39 patients(control group)were diagnosed as non TBL.The clinical characteristics of color Doppler ultrasound,ultrasound elastography and contrast enhanced ultrasound were compared between the two groups.The sensitivity,specificity,positive predictive value,negative predictive value and the area under the receiver operating characteristic(ROC)curve of color Doppler ultrasound,ultrasound elastography,contrast enhanced ultrasound and multimodal ultrasound in the differential diagnosis of superficial tuberculous lymphadenitis were calculated with the pathological test results as the control.Result:The characteristics of color Doppler ultrasound,ultrasound elastography and contrast enhanced ultrasound in the two groups were statistically significant(P<0.05).The AUC of multimodal ultrasound of color Doppler combined with ultrasound elastography was 0.861,and that of color Doppler,ultrasound elastography combined with contrast enhanced multimode ultrasound was 0.935,which was higher than that of color Doppler ultrasound,ultrasound elastography and contrast enhanced multimode ultrasound(P<0.05).The sensitivity,specificity and accuracy of color Doppler ultrasound were 80.39%,66.67%and 74.44%,respectively.The sensitivity,specificity and accuracy of ultrasound elastography were 76.47%,64.10%,and 71.11%,respectively.The sensitivity,specificity and accuracy of contrast enhanced ultrasound were 84.31%,61.54%,and 74.44%,respectively.The sensitivity,specificity and accuracy of multimodal ultrasound of color Doppler ultrasound combined with ultrasound elastography were 90.20%,82.05%and 86.67%respectively.The sensitivity,specificity and accuracy of multimodal ultrasound of color Doppler ultrasound,ultrasound elastography combined with contrast enhanced ultrasound were 92.16%,94.87%and 93.33%,respectively.Conclusion:Multimodal ultrasound has high application value in differential diagnosis of superficial tuberculous lymphadenitis.
