摘要
林寒梅教授认为盆腔炎性不孕症病位在胞宫、冲任、胞脉,病机是正气不足,邪气趁虚而入。正虚以脾肾肝三脏虚损为主。邪实以血瘀为主。正虚日久邪气必侵,邪之所凑,其气必虚,邪实又加重正虚,二者互为因果,使盆腔炎性不孕症病程漫长,病情难愈。治疗以扶正祛邪为原则,扶正以补肾健脾调肝,祛邪以活血化瘀。林寒梅根据“急则治其标,缓则治其本”的原则,在扶正的基础上,重视各种兼证的治疗,根据患者症状体征辨证论治,调周助孕,同时配合中药灌肠,微波,督脉灸等外治法,必要时结合西医腹腔镜手术治疗。提高了疾病治愈率及妊娠率,临床疗效显著。
Professor Lin Hanmei believes that pelvic inflammatory infertility is located in the uterus,thoroughfare and conception channels,and uterine vessels,and its pathogenesis is the defi cient vital qi,which results in the invasion of the pathogenic qi.The deficiency of healthy qi lies mainly in the spleen,kidney and liver,while the pathogen excess mainly features blood stasis.The two factors are mutually causal,making the disease lingering over time and refractory.Its treatment should be based on the principle of strengthening the body resistance and eliminating pathogenic factors,specifi cally and respectively,tonifying the kidney,invigorating the spleen and regulating the liver,and promoting blood circulation and removing blood stasis.On the basis of strengthening the vital qi,Professor Lin Hanmei adheres to the principle of"administering a symptomatic treatment in an acute condition while a radical treatment in a chronic case",attaching importance to various concurrent syndromes,adjusting the menstruation for fostering pregnancy on the basis of differentiating the symptoms and signs of patients.Besides,she supplements her treatment with external therapies such as traditional Chinese medicine enema,microwave,and moxibustion on the governor vessel,or a laparoscopic surgery if necessary.Her practice improves the cure rate and pregnancy rate,achieving a signifi cant clinical eff ect.
