摘要
目的:分析健康冲击对中老年人家庭致贫返贫的影响路径,提出缓解致贫返贫的长效机制。方法:从劳动、情感双重效应路径构建健康冲击对中老年人家庭致贫返贫的作用路径模型,探讨家庭医生签约制度和公共转移支付的调节作用。结果:健康冲击通过减少劳动供给和加剧情感冲击,增加中老年人家庭的医疗支出或债务总额;家庭医生签约制度能满足中老年人情感需求,公共转移支付能弥补健康冲击带来的劳务收入损失,进而改善中老年人家庭致贫返贫问题。结论:政府应坚持积极老龄化理念,发挥家庭医生签约制度的心理干预效果,重视公共转移支付对劳务收入的替代效应,加强中老年人健康风险管理,转换中老年人劳动价值。
Objective To analyze the impact path of health shock on poverty of middle-aged and elderly families,and to propose a long-term mechanism to alleviate poverty.Methods This paper constructs a model of the mechanism of health shock on the poverty of middle-aged and elderly families by the dual effect path of labor and emotion,and discusses the moderating effect of family doctor contract system and public transfer payment.Results Health shock can cause poverty in middle-aged and elderly families,and increase the probability of medical expenditure or total household debt by reducing labor supply and intensifying emotional shock.The family doctor contract system can meet the emotional needs of the elderly.Public transfer payment can make up for the loss of labor service income caused by health shock,and improve poverty of middle-aged and elderly families.Conclusion The government should pay attention to the substitution effect of public transfer payment on labor income,give full play to the psychological intervention effect of the family doctor contract system,strengthen the health risk management of the middle-aged and elderly,and realize the labor value of the middle-aged and elderly.
