摘要
心力衰竭(心衰)是心脏疾病的终末阶段,其病机与“玄府-气液-络脉”密切相关。从临床表现分析,玄府开阖失司是心衰发生的直接原因,气液失和是心衰发生的中心环节,而络脉虚滞是心衰进展的根本原因。从“玄府-气液-络脉”角度探讨心力衰竭的病机,并基于“开通玄府,补虚通络,宣通气液”论治心衰,灵活应用辛味药物、花类药物开玄府,活血化瘀药、利水渗湿药通气液,补益药、虫类药补虚通络,促使瘀、水等有形之邪消散于无形之中,为心衰的中医辨治提供思路。
Heart failure(HF)is the terminal stage of all kinds of heart diseases,and is closely related to the pathology reflected by“sweat pore-qi and liquid-collaterals”.From the perspective of clinical symptoms,the failure of opening and closing of the sweat pores is the direct cause of HF,and the disharmony between qi and fluid is the central of the occurrence,while the deficiency of collaterals is the fundamental reason for the progression of HF.This paper discussed the pathogenesis of HF from the perspective of“sweat pore-qi and liquid-collaterals”,and proposed to treat HF following the principle of“unblocking sweat pores,diffusing and unblocking qi and fluid,supplementing deficiency and unblocking collaterals”by using the acrid medicinals and flower herbs,blood-activating and stasisdissolving medicinals and urination-promoting and dampness-percolating medicinals,as well as supplementing and boosting medicinals and insects,respectively.This can promote the visible pathology such as stasis and water disappearing in the invisible,providing new ideas for the syndrome differentiation and treatment of HF by traditional Chinese medicine.
