摘要
目的:探讨柴苓汤对闭合性多发骨折患者早期血清中肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、降钙素原(PCT)、C反应蛋白(CRP)、白细胞计数(WBC)水平的影响。方法:选取河南省洛阳正骨医院骨盆髋臼外科住院部的闭合性多发骨折患者60例,通过区组随机化分为对照组、中药组,各30例。对照组采取常规复苏治疗,中药组在稳定性复苏后开始服用柴苓汤。统计两组炎性因子(TNF-α、IL-8、IL-6、PCT、CRP、WBC)水平、全身炎症反应综合征(SIRS)及多器官功能障碍综合征(MODS)发生率。结果:稳定性复苏后第1天、第3天两组血清TNF-α、IL-6、IL-8、PCT、CRP、WBC水平比较,差异无统计学意义(P>0.05),第5天、第7天中药组血清TNF-α、IL-6、IL-8、PCT、CRP、WBC水平低于对照组,差异有统计学意义(P<0.05)。中药组SIRS发生率(10.00%)低于对照组(53.33%),差异有统计学意义(P<0.05),中药组MODS发生率(0.00%)与对照组(6.67%)比较,差异无统计学意义(P>0.05)。结论:柴苓汤可有效降低多发骨折伤患者TNF-α、IL-6、IL-8、PCT、CRP、WBC水平,降低SIRS发生率。
Objective:To explore the effect of Chailing decoctionin on the levels of tumor necrosis fractures-α(TNF-α),interleukin-6(IL-6),interleukin-8(IL-8),procalcitonin(PCT),C-reactive protein(CRP)and leukocyte count(WBC)in patients with closed multiple.Methods:Sixty patients with closed multiple fractures in the department of pelvic and acetabular surgery,Luoyang orthopedic hospital,Henan Province,were randomly divided into control group and Chinese medicine group with 30 cases each.The control group was treated with routine resuscitation,and the traditional Chinese medicine group began to take Chailing decoction after stable resuscitation.The levels of inflammatory factors(TNF-α,IL-8,IL-6,PCT,CRP,WBC)and the incidence of systemic inflammatory response syndrome(SIRS)and multiple organ dysfunction syndrome(MODS)in the two groups were calculated.Results:There was no significant difference in the levels of serum TNF-α,IL-6,IL-8,PCT,CRP and WBC between the two groups on the 1st and 3rd day after stable resuscitation(P>0.05).The levels of TNF-α,IL-6,IL-8,PCT,CRP and WBC were lower than those in the control group,and the difference was statistically significant(P<0.05).The incidence of SIRS in the Chinese medicine group(10.00%)was lower than that in the control group(53.33%),and the difference was statistically significant(P<0.05).The incidence of MODS in the Chinese medicine group(0.00%)was not significantly different from that in the control group(6.67%)(P>0.05).Conclusion:Chailing decoction can effectively reduce the levels of TNF-α,IL-6,IL-8,PCT,CRP and WBC in patients with multiple fractures,and reduce the incidence of SIRS.
