摘要
目的探究术前贫血对单侧人工全膝关节置换术(TKA)患者围术期失血量及深静脉血栓(DVT)的影响。方法选取2021年9月~2022年1月在宁夏医科大学总医院因膝关节骨性关节炎(KOA)行单侧TKA的106例患者进行回顾性分析。据患者术前血红蛋白(Hb)水平,将患者分为贫血组(24例)和非贫血组(82例)。比较两组的住院时间、围术期失血量(总失血量以及显、隐性失血量)、围术期DVT、肺栓塞及输血情况。结果贫血组总失血量和隐性失血量均大于非贫血组,差异均有统计学意义(P<0.05);两组显性失血量比较,差异无统计学意义(P>0.05)。贫血组术后DVT发生率及输血率均高于非贫血组,差异均有统计学意义(P<0.05);贫血组患者住院时间长于非贫血组,差异具有统计学意义(P<0.05)。结论术前贫血会导致TKA患者围术期总失血量及隐形失血量增加,术后输血率及术后DVT发生率提高,延长住院时间。
Objective To investigate the effect of preoperative anemia on perioperative blood loss and deep venous thrombosis(DVT)in patients undergoing unilateral total knee arthroplasty(TKA).Methods A retrospective analysis was conducted on 106 patients with knee osteoarthritis(KOA)who underwent unilateral TKA in the General Hospital of Ningxia Medical University from September 2021 to January 2022.According to the preoperative hemoglobin level,the patients were divided into an anemia group(24 cases)and a non-anemia group(82 cases).Hospitalization time,perioperative blood loss,incidences of perioperative DVT and pulmonary embolism,and blood transfusion rate were compared between the two groups.Results Compared with the non-anemia group,the anemia group had more total blood loss and occult blood loss(P<0.05),but no difference was found in the amount of overt blood loss between the two groups(P>0.05).Compared with the non-anemia group,the anemia group had higher incidence of postoperative DVT and higher rate of blood transfusion(P<0.05)and longer hospitalization time(P<0.05).Conclusion In patients undergoing TKA,preoperative anemia may increase total perioperative blood loss and occult blood loss,increase the rate of postoperative blood transfusion and incidence of postoperative DVT,and prolong the hospital stay.
