布地奈德气管内滴入对胎粪吸入综合征气道总氧化剂状态和抗氧化能力的影响

Effect of intratracheal infusion of budesonide on total antioxidant capacity and oxidant status in airway lavage fluids in neonates with meconium aspiration syndrome

摘要 目的研究布地奈德气管内滴入对胎粪吸入综合征新生儿气道灌洗液总氧化剂状态(TOS)和抗氧化能力(TAC)的影响。方法将符合研究条件的62例胎粪吸入综合征新生儿随机分成对照组和研究组,每组31例。对照组给予患儿气管内滴入9 g·L^(-1)氯化钠溶液2mL,研究组给予患者气管内滴入布地奈德2 mL,其他治疗一致。观察两组患者临床结局、并发症、氧疗时间、氧合指数、入院及72 h后气道灌洗液TOS、TAC和氧化应激指数(OSI)。结果研究组患者病死率、并发症、总吸氧时间和氧合指数低于对照组患者,差异没有统计学意义;研究组有创呼吸支持时间显著低于对照组(P=0.031)。入院时两组患者气道灌洗液TOS、TAC和OSI差异没有统计学意义。72 h后,对照组患者气道灌洗液TOS较入院时显著升高(P=0.022),研究组患者灌洗液TOS较入院时显著升高;两组患者气道灌洗液TAC无明显差异;对照组患者OSI显著高于研究组(P<0.001)。结论布地奈德气管内滴入治疗对胎粪吸入综合征可减轻患者气道灌洗液总氧化剂状态和OSI,缩短有创通气时间。 Objective To determine the effect of intratracheal infusion of budesonide on the total antioxidant capacity(TAC)and total oxidant status(TOS)of airway lavage fluids in infants with meconium aspiration syndrome(MAS).Methods Sixty-two neonates with MAS were randomly assigned to a study group(intratracheal infusion of 2 mL budesonide,n=31)and a control group(intratracheal infusion of 2 mL of 9 g·L^(-1) sodium chloride solution,n=31).The airway lavage fluids were obtained both at the admission and 72 hours after the treatment to evaluate the TAC and TOS.Clinical outcomes,complications,the time of oxygen therapy,oxygenation index,TAC and TOS of airway lavage fluid,and oxidative stress index(OSI)in the newborns with MAS in both groups were compared.Results The mortality,total oxygen inhalation time and oxygenation index between the two groups were not significantly different.The invasive respiratory supporting time of the study group was shorter than that of the control group(P=0.031).The TOS,TAC of airway lavage fluids and OSI between the two groups were not significantly different at the admission,while 72 h after the admission,the TOS of airway lavage fluids in the control group was higher than that at the admission(P=0.022).There were no significant changes in the TOS and TAC of airway lavage fluids between the two groups 72 h after the admission.OSI in the control group was higher than that in the study group at 72 h after the admission(P<0.001).Conclusion The TAC of airway lavage fluids,OSI,and the time of invasive ventilation may be reduced in infants with MAS after intratracheal infusion of budesonide.
