摘要
目的通过核因子кB(nuclear factor-кB,NF-кB)信号通路分析参麦注射液联合大黄治疗尿毒症心肌病的效果。方法选取健康成年SPF大鼠72只,造模成功后随机将其分为正常组、假手术组、模型组、益气组(皮下注射参麦注射液)、降浊组(生大黄煎液灌胃)和益气降浊组(皮下注射参麦注射液+生大黄煎液灌胃),每组各12只,干预后处死大鼠,对心肌组织进行Masson染色、苏木精-伊红染色(hematoxylin and eosin staining,HE染色),并比较各组大鼠的左心室重量指数、心脏重量指数、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-6(interleukin-6,IL-6)、NF-κB p65蛋白表达水平。结果益气降浊组大鼠的左心室重量指数、心脏重量指数、TNF-α、IL-6及NF-κB p65蛋白表达水平均显著低于模型组、益气组、降浊组、假手术组(P<0.05)。益气降浊组大鼠的心肌纤维走行相同,且心肌组织的分布均匀性、细胞排列整齐度均优于益气组和降浊组。益气降浊组大鼠的心肌胶原沉积均明显减少,心肌纤维整齐,变化最为明显。结论尿毒症可通过NF-кB信号通路过度激活而诱发心肌损伤,参麦注射液联合大黄能抑制该信号通路过度激活,缓解炎症反应,保护心肌细胞,预防尿毒症心肌病。
Objective To analyze the effect of Shenmai injection combined with rhubarb in the treatment of uremic cardiomyopathy through nuclear factor-кB(NF-кB)signaling pathway.Methods Seventy-two healthy SPF rats were selected and randomly divided into normal group,sham operation group,model group,Yiqi group(subcutaneous injection of Shenmai injection),Jiangzhuo group(gavage of rhubarb decoction),and Yiqi Jiangzhuo group(subcutaneous injection of Shenmai injection+gavage of rhubarb decoction),with 12 rats in each group.After the intervention,rats were killed,and myocardial tissue was stained by Masson staining and hematoxylin and eosin staining(HE staining).The left ventricular mass index,heart mass index,tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),and NF-κB p65 protein expression levels of all groups were compared.Results The left ventricular mass index,heart mass index,TNF-α,IL-6 and NF-κB p65 protein expression levels in Yiqi Jiangzhuo group were lower than those in model group,Yiqi group,Jiangzhuo group,and sham operation group(P<0.05).The myocardial fibers of Yiqi Jiangzhuo group were the same,and the distribution uniformity of myocardial tissue and the uniformity of cell arrangement were better than those of Yiqi group and Jiangzhuo group.Myocardial collagen deposition was significantly reduced in the Yiqi and turbidity reducing group,and the myocardial fibers were neat,with the most obvious changes.Conclusion Uremia can induce myocardial damage through the over-activation of NF-кB signaling pathway,and Shenmai injection combined with rhubarb can inhibit the over-activation of this signaling pathway,relieve inflammatory response,protect cardiomyocytes,and prevent uremic cardiomyopathy.
