摘要
目的 探讨宫颈经阴道剪切波弹性成像(SWE)联合肿瘤标志物血清鳞状细胞癌抗原(SCCA)定量诊断宫颈癌的临床价值。方法 选取拟诊为宫颈癌的患者120例,行常规阴道超声、SWE及SCC-Ag检查,以病理结果评定为诊断的金标准,绘制ROC曲线评价宫颈弹性模量平均值(Emean)、SCC-Ag及两者联合定量诊断宫颈癌的价值。结果 120例患者病理结果诊断为宫颈癌55例(恶性组),良性病变65例(良性组),两组间年龄、体质指数(BMI)的差异无统计学意义,恶性组Emean(62.21±2.60) kPa、SCC-Ag(6.89±1.82) ng/ml均高于良性组(36.76±1.97) kPa、(2.02±1.31) ng/ml,差异均有统计学意义(均P<0.05)。以Emean≥39.9 kPa且SCC-Ag≥2.38 ng/ml为最佳截断值,两者联合预测宫颈癌的AUC为0.926[95%CI(0.868,0.983)],灵敏度为93.0%,特异性为95.0%。结论 SWE、SCC-Ag对定量诊断宫颈癌均有一定的价值,但两者联合定量诊断灵敏度、特异性更高。
Objective To investigate the clinical value of cervical transvaginal shear wave elastography(SWE) combined with tumor marker serum squamous cell carcinoma antigen(SCC-Ag) in the quantitative diagnosis of cervical cancer. Methods 120 patients with cervical cancer in our hospital were examined by clinical information collection, routine vaginal ultrasound, SWE and SCC-Ag. The pathological results were evaluated as the gold standard for diagnosis, and the ROC curve was drawn to evaluate the value of cervical elastic modulus average(Emean), SCC-Ag alone and their combination in the quantitative diagnosis of cervical cancer. Results The pathological results of 120 patients showed that there were 55 cases of cervical cancer(malignant group) and 65 cases of benign lesions(benign group). There was no significant difference in age and body mass in dex(BMI) between the two groups. Emean(62.21±2.60) kPa and SCC-Ag(6.89±1.82) ng/ml in malignant group were higher than those in benign group(36.76±1.97) kPa and(2.02±1.31) ng/ml(all P<0.05). Taking Emean ≥ 39.9 kPa and SCC-Ag ≥ 2.38 ng/ml as the best cut-off value, the AUC of the combined prediction of cervical cancer was 0.926[95%CI(0.868, 0.983)], with a sensitivity of 93.0% and a specificity of 95.0%. Conclusion SWE and SCC-Ag are valuable in the quantitative diagnosis of cervical cancer, but the combination of SWE and SCCA has higher sensitivity and specificity.
