摘要
目的分析同型半胱氨酸(Hcy)和脂蛋白相关磷脂酶A2(Lp-PLA2)水平在急性缺血性脑卒中患者的变化。方法便利选取2018年7月—2020年1月厦门市第五医院收治的急性缺血性脑卒中患者127例作为研究组,采用美国国立卫生院卒中量表(NIHSS)评价患者的神经功能缺损情况,根据评分差异分为研究1组(41例,<4分、轻度缺损)、研究2组(56例,4~15分、中度缺损)、研究3组(30例,>15分、重度缺损),另选取同时间段于本院行健康体检者55名作为对照组,所有受检者均检测其Hcy、Lp-PLA2和肝肾功能等相关指标,分析Hcy和Lp-PLA2在急性缺血性脑卒中患者中的变化。结果研究组Hcy、Lp-PLA2、超敏C反应蛋白(hs-CRP)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白(LDL)、肌酐和尿酸水平均高于对照组,高密度脂蛋白(HDL)和尿素氮水平均低于对照组,差异有统计学意义(P<0.05)。研究1组Hcy(21.58±4.98)μmol/L和Lp-PLA2(221.71±15.42)ng/mL均低于研究2组[(24.66±5.05)μmol/L和(248.05±15.67)ng/mL]和研究3组[(29.89±5.34)μmol/L和(259.24±15.99)ng/mL],差异有统计学意义(F=23.140、56.720,P<0.05)。结论Hcy和Lp-PLA2水平在急性缺血性脑卒中患者中呈高表达状态,患者病情的严重程度与两项指标的水平高低关系密切,并且神经功能缺损情况越严重的患者,Hcy和Lp-PLA2水平越高,因此在临床对该疾病的诊断和治疗效果中可参考这两项指标的水平。
Objective To analyze the changes of homocysteine(Hcy)and lipoprotein-associated phospholipase A2(Lp-PLA2)levels in patients with acute ischemic stroke.Methods Convenient selection a total of 127 patients with acute ischemic stroke admitted to the Xiamen Fifth Hospital from July 2018 to January 2020 were selected as the study group.The National Institutes of Health Stroke Scale(NIHSS)was used to evaluate the neurological deficit of the patients,and they were divided into study group 1(41 cases,<4 points,mild defect)and study group 2(56 cases,4-15 points,moderate defect)and study group 3(30 cases,>15 points,severe defect)according to the difference in scores.In addition,55 people who underwent physical examination in the hospital during the same time period were selected as the control group.All subjects were tested for Hcy,Lp-PLA2,liver and kidney function and other related indicators,and the changes of Hcy and Lp-PLA2 in patients with acute ischemic stroke were analyzed.Results The levels of Hcy,Lp-PLA2,high-sensitivity C-reactive protein(hs-CRP),total cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL),creatinine and uric acid in the study group were higher than those in the control group,high density lipoprotein(HDL)and blood urea nitrogen levels in the study group were lower than those in the control group,the difference was statistically significant(P<0.05).Hcy(21.58±4.98)μmol/L and Lp-PLA2(221.71±15.42)ng/mL in study group 1 were lower than[(24.66±5.05)μmol/L and(248.05±15.67)ng/mL]in study group 2 and[(29.89±5.34)μmol/L and(259.24±15.99)ng/mL]in the study group 3,the difference was statistically significant(F=23.140,56.720,P<0.05).Conclusion The levels of Hcy and Lp-PLA2 are highly expressed in patients with acute ischemic stroke,and the severity of the patient′s condition is closely related to the levels of the two indicators.The higher the level of PLA2,the combined detection of the two indicators can be referred to in the clinical diagnosis of the disease and the judgment of the treatment effect.
