摘要
目的:探讨黄芪桂枝五物汤对缓慢性心律失常患者心率恢复、血清脂肪酸结合蛋白及血浆血管紧张素Ⅱ(AngⅡ)、Adropin、半乳糖凝集素(Gal)3的影响。方法:选择2020年4月至2021年4月该院收治的120例缓慢性心律失常患者,按随机数字表法分组,60例对照组患者给予常规治疗,60例观察组患者在对照组的基础上给予黄芪桂枝五物汤治疗。治疗前后检测两组患者的Gal-3、血管活性肠肽(VIP)、一氧化氮(NO)、AngⅡ、Adropin、生长分化因子(GDF)15、肌酸激酶同工酶(CK-MB)、心型脂肪酸结合蛋白(H-FABP)、内源性孤啡肽(N/OFQ)、左心室射血分数(LVEF)、心率、相邻NN间期差值的均方根(RMSSD)、每5 min NN间期平均值标准差(SDANN)及全部窦性心搏NN间期(SDNN)水平,比较两组患者的临床疗效。结果:治疗后,观察组患者的AngⅡ水平低于对照组,VIP、Adropin和NO水平高于对照组,Gal-3、GDF-15水平低于对照组,H-FABP、CK-MB和N/OFQ水平低于对照组,LVEF、心率水平高于对照组,RMSSD、SDANN水平低于对照组,SDNN水平高于对照组,差异均有统计学意义(P<0.05)。观察组患者的总有效率为96.67%(58/60),高于对照组的83.33%(50/60),差异有统计学意义(P<0.05)。结论:黄芪桂枝五物汤用于缓慢性心律失常患者的治疗,可改善其血管内皮功能,抑制机体炎症,减少心肌损伤,改善心率、心率变异性及心功能,提高临床疗效。
OBJECTIVE: To probe into the effects of Huangqi Guizhi Wuwu decoction on heart rate recovery from bradyarrhythmia, serum fatty acid binding protein levels and plasma Ang Ⅱ, Adropin and galectin(Gal) 3. METHODS: A total of 120 patients with bradyarrhythmias admitted into the hospital from Apr. 2020 to Apr. 2021 were extracted to be divided into the control group and the observation group via the random number table method, with 60 cases in each group. The control group was treated with routine treatment, while the observation group received Huangqi Guizhi Wuwu decoction based on the control group. The levels of Gal-3, vasoactive intestinal peptide(VIP), nitric oxide(NO) and Ang Ⅱ, Adropin, growth differentiation factor(GDF)-15, creatine kinase isoenzyme(CK-MB), heart-type fatty acid binding protein(H-FABP), endogenous nociceptin/orphanin FQ(N/OFQ), left ventricular ejection fraction(LVEF), heart rate(HR), root mean square of the difference between successive NN intervals(RMSSD), every 5 min NN standard deviation of mean interval(SDANN) and total NN interval(SDNN) before and after treatment in two groups were detected. Clinical efficacy of two groups were compared. RESULTS: After treatment, the level of Ang Ⅱ of the observation group was lower than that of the control group, the levels of VIP, Adropin and NO were higher than those of the control group, the levels of Gal-3, GDF-15 was lower than that of the control group, the levels of H-FABP, CK-MB and N/OFQ were lower than those of the control group, the levels of LVEF and HR were higher than those of the control group, the levels of RMSSD and SDANN were lower than those in the control group, the level of SDNN was higher than that of the control group, with statistically significant differences(P<0.05). The total effective rate of the observation group was 96.67%(58/60), higher than 83.33%(50/60) of the control group, the difference was statistically significant(P<0.05). CONCLUSIONS: Huangqi Guizhi Wuwu decoction in the treatment of patients with bradyarrhythmias can improve vascular endothelial function, inhibit inflammation, reduce myocardial damage, improve heart rate, heart rate variability and cardiac function, and improve the clinical efficacy.
