摘要
目的探究不同麻醉方式在老年髋部骨折患者中的麻醉效果及对应激反应和免疫水平的影响。方法选取2020年1月至2021年12月我院收治的105例老年髋部骨折患者,分成3组,接受全身麻醉的患者为A组,接受神经阻滞麻醉的患者为B组,接受腰硬联合麻醉的患者为C组。结果与A组比,B组、C组的麻醉效果更优(P<0.05),与B组比,C组的麻醉效果更优(P<0.05);麻醉后12 h,与A组比,B组、C组的应激反应指标均更优(P<0.05),而与B组比,C组的应激反应指标更优(P<0.05);麻醉后12 h,与A组比,B组、C组的免疫球蛋白指标均更优(P<0.05),而与B组比,C组的免疫球蛋白指标更优(P<0.05);B组与C组的不良反应发生率差异无统计学意义(P>0.05),与A组比,B组与C组的不良反应发生率更低(P<0.05)。结论在老年髋部骨折患者手术治疗过程中应用合理化的麻醉方式极为关键,3种麻醉方式均存在优缺点,但对于老年患者而言,腰硬联合麻醉的整体效果更优,应激反应较小,短时间内可以基本恢复免疫功能,提高治疗的安全性,值得推广应用。
Objective To explore the effects of different anesthesia methods on stress response and immune level in elderly patients with hip fractures.Methods A total of 105 elderly patients with hip fractures admitted to our hospital from January 2020 to December 2021 were randomly divided into three groups:group A receiving general anesthesia,group B receiving nerve block anesthesia,and group C receiving spinal combined spinal epidural anesthesia.Results Compared with group A,the anesthetic effect of groups B and C was better(P<0.05),and compared with group B,the anesthetic effect of group C was better(P<0.05).12 h after anesthesia,compared with group A,the stress response indexes of groups B and C were better(P<0.05),and compared with group B,the stress response indexes of group C were better(P<0.05).At 12 h after anesthesia,the immunoglobulin index of groups B and C was better than that of group A(P<0.05),and the immunoglobulin index of group C was better than that of group B(P<0.05).There was no significant difference in the incidence of adverse reactions between groups B and C(P>0.05).Compared with group A,the incidence of adverse reactions in groups B and C was lower(P<0.05).Conclusion In elderly patients with hip fracture surgery,applications in the process of rationalization of anesthesia appropriate way is extremely critical,there are pros and cons to three anesthesia way,but for elderly patients,the spinal canal waist hard joint anesthesia has a better overall effect and less stress,and can restore immune function in a short period of time,and is worthy of popularization and application.
