摘要
目的研究股骨粗隆间骨折(intertrochanteric fracture of femur,IFOF)老年患者应用股骨近端防旋髓内钉(proximal femoral nail antirotation,PFNA)内固定术联合加味桃红四物汤治疗的效果。方法选取2019年6月—2021年12月江苏省如皋市中医院收治的114例IFOF老年患者为研究对象,按随机数字表法分为两组,各57例。对照组行PFNA内固定术治疗,研究组行PFNA内固定术联合加味桃红四物汤治疗,对比两组恢复情况、疼痛度及髋关节功能。结果与对照组比较,研究组骨折愈合时间、患肢消肿时间均更短,差异有统计学意义(P<0.05);研究组术后1周、术后4周及术后8周Harris评分分别为(64.62±7.18)分、(76.78±6.48)分、(85.59±4.44)分,均高于对照组(51.29±7.48)分、(67.32±5.26)分、(79.58±4.18)分,差异有统计学意义(t=9.706、8.557、7.440,P<0.05);研究组术后3 d、术后5 d及术后7 d VAS评分均低于对照组,差异有统计学意义(t=8.314、16.805、7.069,P<0.05)。结论IFOF老年患者应用PFNA内固定术联合加味桃红四物汤治疗,可减轻术后疼痛度,缩短患者恢复时间,并改善髋关节功能。
Objective To study the effect of internal fixation with Proximal Femoral Nail Antirotation(PFNA)in combination with Taohongsiwu Decoction in elderly patients with Intertrochanteric fracture of femur(IFOF).Methods 114 elderly patients with IFOF admitted to Rugao Hospital of Traditional Chinese Medicine in Jiangsu Province from June 2019 to December 2021 were selected as the study subjects.They were divided into two groups,with 57 cases in each.The control group was treated with PFNA internal fixation,and the study group was treated with PFNA internal fixation combined with the addition of Taohongsiwu Decoction.The recovery,pain level and hip function were compared between the two groups.Results Compared with the control group,the fracture healing time and swelling time of the affected limb were shorter in the study group,the difference was statistically significant(P<0.05).The Harris scores at 1 week,4 weeks and 8 weeks postoperatively were(64.62±7.18)points,(76.78±6.48)points and(85.59±4.44)points in the study group,respectively,which were higher than those in the control group(51.29±7.48)points,(67.32±5.26)points and(79.58±4.18)points,the difference was statistically significant(t=9.706,8.557,7.440,P<0.05).The VAS scores in the study group were lower than those in the control group at 3 d,5 d and 7 d postoperatively,the difference was statistically significant(t=8.314,16.805,7.069,P<0.05).Conclusion The application of PFNA internal fixation combined with addition of Taohongsiwu Decoction in elderly patients with IFOF can reduce the postoperative pain degree,shorten the recovery time of patients and improve the function of hip joint.
