摘要
目的观察和比较人类视网膜脱离后视网膜不同部位细胞的死亡方式及形态学变化。方法在5名孔源性视网膜脱离患者的手术过程中分别取孔盖、原发孔后缘、术中造孔后缘的视网膜组织,制片后分别记为A、B、C组标本,采用电子显微镜观察各组视网膜细胞的死亡方式和形态学变化,及线粒体、内质网的形态学变化。结果观察A组标本,视网膜脱离后,细胞表现为凋亡和坏死性凋亡,以凋亡细胞居多,未发现典型的坏死细胞,可见线粒体空泡化,内部结构丢失,内质网略膨大;B组标本中细胞出现坏死、凋亡、坏死性凋亡的表现,以坏死现象最典型,可以发现视锥细胞中的线粒体出现肿胀、嵴断裂和内部结构丢失,在神经纤维层旁发现正常的内质网结构;C组标本中发现组织中存在大量正常细胞,偶见凋亡和坏死细胞,未发现坏死性凋亡细胞,单纯的自噬现象更加多见;3组标本的观察结果进行对比,结果显示A、B、C 3组标本细胞死亡的程度依次减轻,正常细胞数量依次增加。结论视网膜脱离后,视网膜细胞出现凋亡、坏死、坏死性凋亡以及自噬的形态学改变;距离原发病灶越近的视网膜,其细胞的死亡程度越重,细胞数量越少。
Objective To observe and compare the death mode and morphological changes of cells in different parts of retina after retinal detachment.Methods It was a prospective study.The retinal tissues of the hole cover,the posterior edge of the retinal hole and the posterior edge of the retinotomy were taken respectively,and recorded as A group,B group and C group samples in the operation of 5 patients with rhegmatogenous retinal detachment.The death mode and morphological changes of retinal cells,as well as the morphological changes of mitochondria and endoplasmic reticulum were observed by electron microscope.Resultss In A group,the cells showed apoptosis and necrotic apoptosis,most of which were apoptotic cells,and no typical necrotic cells were found.Mitochondria vacuolization,loss of internal structure and slight expansion of endoplasmic reticulum were observed in the cells.In B group.Necrosis,apoptosis and necrotic apoptosis appeared in the cells,and necrosis was the most typical.Swelling,cristae rupture and internal structure loss of mitochondria in cones could be found,and normal endoplasmic reticulum structure was found next to the nerve fiber layer.In addition to a few apoptotic and necrotic cells,a large number of normal cells were found in the tissues of C group,no necrotic apoptotic cells were found,and simple autophagy was more common.Comparing the specimens of three groups,the results showed that the degree of cell death of A,B and C groups decreased in turn,and the number of normal cells gradually increased.Conclusions After retinal detachment,apoptosis,necrosis,necrotic apoptosis and autophagy were found in retinal cells.The retina closer to the primary retinal hole had heavier cell death and fewer normal cell numbers.
