甲泼尼龙联合阿奇霉素治疗小儿难治性支原体肺炎的临床效果 被引量:11

摘要 目的探究甲泼尼龙联合阿奇霉素治疗小儿难治性支原体肺炎的临床效果。方法将我院收治的96例难治性支原体肺炎患儿按照治疗方法的不同分为对照组(48例,在常规治疗的基础上给予阿奇霉素)和观察组(48例,给予甲泼尼龙联合阿奇霉素)。比较两组的治疗效果。结果观察组的发热、咳嗽、肺部啰音消失时间均短于对照组(P<0.05)。治疗后,观察组的动脉血氧分压(PaO_(2))、用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))高于对照组,动脉血二氧化碳分压(PaCO_(2))低于对照组(P<0.05)。治疗后,观察组的C-反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、降钙素原(PCT)水平及白细胞计数(WBC)、中性粒细胞百分比(NE%)低于对照组(P<0.05)。观察组的治疗总有效率高于对照组(P<0.05)。两组的不良反应总发生率无显著差异(P>0.05)。结论甲泼尼龙联合阿奇霉素治疗小儿难治性支原体肺炎安全、有效。 Objective To explore the clinical effect of methylprednisolone combined with azithromycin in the treatment of refractory mycoplasma pneumonia in children.Methods A total of 96 children with refractory mycoplasma pneumonia in our hospital were divided into control group(48 cases,given azithromycin on the basis of conventional treatment)and observation group(48 cases,given methylprednisolone combined with azithromycin)according to different treatment methods.The therapeutic effects of the two groups were compared.Results The disappearance time of fever,cough and lung rale in the observation group were shorter than those in the control group(P<0.05).After treatment,the arterial partial pressure of oxygen(PaO_(2)),forced vital capacity(FVC)and forced expiratory volume in first second(FEV_(1))in the observation group were higher than those in the control group,the arterial partial pressure of carbon dioxide(PaCO_(2))was lower than that in the control group(P<0.05).After treatment,the levels of C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),procalcitonin(PCT),white blood cell count(WBC)and percentage of neutrophil(NE%)in the observation group were lower than those in the control group(P<0.05).The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Methylprednisolone combined with azithromycin is safe and effective in the treatment of refractory mycoplasma pneumonia in children.
