摘要
目的:探讨中药外敷包温热敷下腹部治疗阴道分娩产后子宫复旧不良的临床效果。方法:选取收治的137例阴道分娩产后子宫复旧不良的产妇作为研究对象,随机分为对照组(n=69)与观察组(n=68),对照组予以缩宫素喷鼻剂治疗,观察组予以缩宫素喷鼻剂联合中药外敷包治疗,比较两组治疗7 d后临床疗效、临床症状(子宫压痛、阴道流血、腰腹重坠)消失时间、子宫横径、长径、前后径、三径之和;治疗前及治疗1 d、2 d、3 d、4 d、5 d、6 d、7 d子宫宫缩疼痛程度及治疗期间不良反应发生情况。结果:治疗7 d后,观察组临床疗效高于对照组;观察组临床症状消失时间均短于对照组;观察组子宫横径、长径、前后径、三径之和均小于对照组;治疗1 d、2 d、3 d、4 d、5 d、6 d、7 d后观察组VAS评分低于对照组,差异均有统计学意义(均P<0.05),且治疗期间两组均未发生明显不良反应。结论:中药外敷包热敷下腹部可有效缓解子宫复旧不良产妇产后宫缩痛,促进子宫复旧。
Objective To observe the clinical effect of warm and hot compress of traditional Chinese medicine on the lower abdomen in the treatment of uterine involution after vaginal delivery.Method A total of 137 puerperae with poor uterine involution after vaginal delivery who were treated were selected as the research objects,and they were randomly divided into control group(n=69)and observation group(n=68),the control group was treated with oxytocin nasal spray,the observation group was treated with oxytocin nasal spray combined with traditional Chinese medicine external application pack,and the clinical efficacy,the disappearance time of clinical symptoms(uterine tenderness,vaginal bleeding,waist and abdomen),the transverse uterus were compared between the two groups after 7 days of treatment.Diameter,long diameter,anteroposterior diameter,sum of three diameters;before treatment and 1 d,2 d,3 d,4 d,5 d,6 d,7 d of uterine contraction pain and the occurrence of adverse reactions during treatment.Results After 7 days of treatment,the clinical efficacy of the observation group was higher than that of the control group(P<0.05);the disappearance time of clinical symptoms of the observation group was shorter than that of the control group(P<0.05);The sum of transverse diameter,longitudinal diameter,anteroposterior diameter and three diameters of uterus in the observation group were smaller than those in the control group(P<0.05);the VAS score of the observation group was lower than that of the control group after 1 d,2 d,3 d,4 d,5 d,6 d,and 7 d of treatment(P<0.05),and no obvious adverse reaction occurred in both groups during the treatment.Conclusion The traditional Chinese medicine external application package hot compress on the lower abdomen can effectively relieve postpartum uterine contraction pain and promote uterine involution in poor uterine involution.
