摘要
目的观察^(125)I粒子植入与间歇性内分泌联合治疗对中晚期前列腺癌临床疗效,以及对患者癌细胞PC-3、血管内皮生长因子(VEGF)的影响。方法选取2019年3月至2021年5月收治的54例晚期前列腺癌患者为研究对象,根据治疗方法的不同随机分为观察组和对照组,每组27例。对照组患者采用间歇性内分泌治疗(IHT),观察组患者采用^(125)I粒子植入与间歇性内分泌联合治疗。比较2组治疗2个月后的临床疗效、国际前列腺症状评分(IPSS)、最大尿流率(Qmax)、前列腺体积(PV)、前列腺特异性抗原(PSA)、癌细胞PC-3凋亡率及VEGF水平的变化,观察2组治疗期间不良反应的发生率。结果2组患者治疗后,观察组患者治疗有效率(77.78%)明显高于对照组治疗有效率(44.44%),组间比较具有统计学意义(P<0.05)。2组患者治疗前IPSS、Qmax、PV、PSA水平,组间比较无统计学意义(P>0.05),治疗后观察组患者IPSS、PV、PSA水平低于对照组,Qmax水平高于对照组,组间比较具有统计学意义(P<0.05)。2组患者治疗后癌细胞PC-3细胞凋亡率较前明显升高、而VEGF水平较治疗前比较显著降低,且观察组较对照组上述指标改善更明显(P<0.05),组间比较具有统计学意义(P<0.05)。2组患者治疗期间不良反应发生率,观察组显著低于对照组,组间比较具有统计学意义(P<0.05)。结论^(125)I粒子植入与间歇性内分泌联合治疗晚期前列腺癌疗效显著,还能抑制癌细胞PC-3的增殖速度和病灶内血管的生长速度,且不良反应发生率低,具有一定的临床意义。
Objective To investigate the therapeutic efficacy of^(125)I radioactive particle implantation combined with intermittent hormonal therapy(IHT)on advanced prostate cancer,and the regulatory effect on the apoptosis of PC-3 cells and expression level of vascular endothelial growth factor(VEGF).Methods A total of 54 advanced prostate cancer patients treated in our center from March 2019 to May 2021 were recruited and randomly assigned into observation group(n=27)and control group(n=27).All patients were managed by IHT,and those in observation group were additionally treated with^(125)I radioactive particle implantation.After 2 months of the treatment,therapeutic efficacy,the International Prostate Symptom Score(IPSS),the maximum urine flow rate(Qmax),prostate volume(PV),prostate-specific antigen(PSA)level,apoptosis rate of PC-3 cells and expression level of VEGF between groups were compared.In addition,adverse events in both groups were recorded.Results After treatment,the overall therapeutic efficacy of observation group was significantly higher than that of control group(P<0.05).No significant differences in IPSS,Qmax,PV and PSA level before treatment were detected between groups(P>0.05).After treatment,significantly lower IPSS,PV and PSA level,and higher Qmax were detected in observation group than those of control group(P<0.05).Increased apoptosis rate of PC-3 cells and decreased expression level of VEGF were measured in both groups after treatment,which were significantly pronounced in observation group(both P<0.05).The incidence of adverse events was significantly lower in observation group than that of control group(P<0.05).Conclusion The combination therapy of^(125)I radioactive particle implantation and IHT in advanced prostate cancer patients is highly effective and safe.It also inhibits the proliferation of prostate cancer cells and the angiogenesis in cancer lesions,with certain clinically significant.
