不同酶免检测试剂及胶体硒法在HIV初筛中的应用效果 被引量:1

摘要 目的探讨不同酶免检测试剂及胶体硒法在人类免疫缺陷病毒(HIV)初筛中的应用效果。方法选取2016年1月至2020年1月150例具有HIV高危感染风险而接受HIV抗体筛查的疑似HIV感染患者作为研究对象,分别采用第三代酶免检测试剂、第四代酶免检测试剂、胶体硒法进行HIV抗体检测。以免疫印迹法结果为金标准,比较胶体硒法、第三代酶免检测试剂、第四代酶免检测试剂对HIV抗体的诊断灵敏度、特异度、准确度、阳性预测值、阴性预测值;分析胶体硒法、第三代酶免检测试剂、第四代酶免检测试剂诊断结果与免疫印迹法诊断结果的一致性。结果150例疑似HIV感染患者中,经免疫印迹法检测,有96例证实为HIV抗体阳性,54例证实为HIV抗体阴性。第四代酶免检测试剂的灵敏度、特异度、准确度、阳性预测值、阴性预测值高于胶体硒法和第三代酶免检测试剂(P<0.05);第三代酶免检测试剂与胶体硒法的灵敏度、特异度、准确度、阳性预测值、阴性预测值比较,差异无统计学意义(P>0.05)。胶体硒法诊断结果与免疫印迹法诊断结果之间具有中度一致性(Kappa值=0.539);第三代酶免检测试剂诊断结果与免疫印迹法诊断结果之间具有中度一致性(Kappa值=0.621);第四代酶免检测试剂诊断结果与免疫印迹法诊断结果之间具有高度一致性(Kappa值=0.872)。结论在HIV初筛中,酶联免疫吸附法对HIV抗体阳性的筛查准确性好,尤其是第四代酶免检测试剂,可进一步提高HIV抗体阳性的检出率。 Objective To investigate the application effect of different enzyme immunoassay reagents and colloidal selenium method in human immunodeficiency virus(HIV)primary screening.Methods From January 2016 to January 2020,150 suspected HIV-infected patients with high risk of HIV infection who received HIV antibody screening were selected as the research objects,HIV antibody was detected by the third generation enzyme immunoassay reagent,the fourth generation enzyme immunoassay reagent and colloidal selenium method.The results of immunoblotting method were used as the gold standard,the diagnostic sensitivity,specificity,accuracy,positive predictive value and negative predictive value of colloidal selenium method,the third generation enzyme immunoassay reagent and the fourth generation enzyme immunoassay reagent for HIV antibody were compared;the consistency of the diagnostic results of colloidal selenium method,the third generation enzyme immunoassay reagent and the fourth generation enzyme immunoassay reagent with the diagnostic results of immunoblotting method was analyzed.Results Among 150 patients with suspected HIV infection,96 cases were confirmed to be HIV antibody positive and 54 cases were confirmed to be HIV antibody negative by immunoblotting method.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of the fourth generation enzyme immunoassay reagent were higher than those of the colloidal selenium method and the third generation enzyme immunoassay reagent(P<0.05);there were no significant differences in sensitivity,specificity,accuracy,positive predictive value and negative predictive value between the third generation enzyme immunoassay reagent and the colloidal selenium method(P>0.05).There was moderate consistency between the diagnostic results of the colloidal selenium method and the diagnostic results of immunoblotting method(Kappa value=0.539);there was moderate consistency between the diagnostic results of the third generation enzyme immunoassay reagent and the diagnostic results of immunoblotting method(Kappa value=0.621);there was high degree of consistency between the diagnostic results of the fourth generation enzyme immunoassay reagent and the diagnostic results of immunoblotting method(Kappa value=0.872).Conclusion In the primary screening of HIV,the accuracy of enzyme-linked immunosorbent assay for HIV antibody positive screening is good,especially the fourth generation enzyme immunoassay reagent,which can further improve the detection rate of HIV antibody positive.
