摘要
目的探讨舌下脱敏治疗低龄儿童过敏性鼻炎(AR)的临床价值。方法选取2020年1月至2021年12月广西医科大学附属武鸣医院592例3~8岁低龄AR患儿,采用随机数字表法分为脱敏组与对照组,每组各296例。对照组接受西替利嗪与氮卓斯汀鼻喷雾治疗,脱敏组在对照组基础上舌下含服尘螨滴剂治疗。比较两组的症状改善时间、血清免疫球蛋白A(IgA)、免疫球蛋白E(IgE)、Lund-Kennedy内镜评分、血清C反应蛋白(CRP)及不良反应。结果脱敏组治疗后双侧Lund-Kennedy内镜评分与血清IgA、CRP、IgE水平低于对照组(P<0.05)。脱敏组症状改善时间短于对照组(P<0.05)。脱敏组与对照组不良反应总发生率(4.75%vs.4.08%)比较,差异无统计学意义(P>0.05)。结论舌下脱敏治疗能够改善低龄AR患儿鼻黏膜炎症与水肿症状,改善血清免疫指标与炎症因子水平。
Objective To investigate the clinical value of sublingual immunotherapy in young children with allergic rhinitis(AR).Methods A total of 592 AR children aged 3-8 years in Wuming Hospital of Guangxi Medical University from January 2020 to December 2021 were selected and divided into desensitization group and control group by random number table method,with 296 cases in each group.The control group was treated with Cetirizine and Azelastine nasal spray,and the desensitization group was treated with Dust Mite Drops under the tongue on the basis of the control group.The symptom improvement time,serum immunoglobulin A(IgA),immunoglobulin E(IgE),Lund-Kennedy endoscopic score,serum C-reactive protein(CRP)and adverse reactions were compared between the two groups.Results The bilateral Lund-Kennedy endoscopic scores and serum IgA,CRP and IgE levels in the desensitization group after treatment were lower than those in the control group(P<0.05).The improvement time of symptoms in desensitization group was shorter than that in control group(P<0.05).There was no significant difference in the total incidence of adverse reaction rate between desensitization group and control group(4.75%vs.4.08%)(P>0.05).Conclusion Sublingual immunotherapy can improve the symptoms of nasal mucosa inflammation and edema,and improve serum immune indicators and inflammatory factors in in young children with AR.
