摘要
目的优化肿瘤专科医院临时采购药品的审批流程。方法选取某肿瘤专科医院医院信息系统中2019年9月1日至2020年8月31日(干预前组)及2020年9月1日至2021年8月31日(干预后组)完成OA审批的临时采购药品计划单,干预后组在审批过程中引入简易量化评分表。从安全性、有效性、经济性、适宜性、必要性5个药品属性,以及采购品种(国家组织药品集中带量采购、湖南省采购平台省际联动/备选)、是否《国家基本药物目录》品种、是否基本医疗保险药品3个政策因素方面进行评分。结果干预前组临时采购药品共85个品规,其中抗肿瘤药物30个、抗菌药物8个、其他辅助用药47个;干预后组共69个品规,其中抗肿瘤药物25个、抗菌药物11个、辅助用药33个。干预后组临时采购药品的平均综合评分为7.59分,显著高于干预前组的7.15分(P<0.05)。干预后,有同类可替代、价格优势不显著等临时采购品种有所减少,但其中仍包含多个国家集中带量采购、湖南省集中带量采购药品的同通用名药品。结论简易量化评分表有利于规范肿瘤专科医院临时采购药品的审批流程,保障采购药品的高品质、合理性和必要性。医疗机构在配合医保部门统筹协调院内药品采购、使用管理制度的同时,还应积极探索保障患者可负担性和多重选择性的药品管理方法。
Objective To optimize the approval process of drugs temporarily procured in the cancer hospitals.Methods The plans of drugs temporarily procured approved by the OA system were selected from the hospital information system of a cancer hospital from September 1,2019 to August 31,2020(pre-intervention group)and from September 1,2020 to August 31,2021(post-intervention group).In the post-intervention group,a simple quantitative scoring table was introduced in the approval process to score the drugs temporarily procured from five drug attributes(safety,effectiveness,economy,suitability and necessity)and three policy factors[procurement varieties(through the national centralized drug-procurement or inter-provincial linkage/alternative procurement by the procurement platform of Hunan Province),varieties of National Essential Drugs List or not and drugs for basic medical insurance or not].Results A total of 85 drug specifications were temporarily procured in the pre-intervention group,including 30 antitumor drugs,eight antibacterial drugs and 47 adjuvant drugs,while 69 drug specifications were temporarily procured in the post-intervention group,including 25 antitumor drugs,11 antibacterial drugs and 33 adjuvant drugs.The average comprehensive score of drugs temporarily procured in the post-intervention group was 7.59 points,which was significantly higher than 7.15 points in the pre-intervention group(P<0.05).After the intervention,the varities of temporarily procured with similar substitutability and insignificant economic advantages were less,but there were still many drugs with the same generic names through the national centralized drug-procurement or centralized drug-procurement by Hunan Province.Conclusion The simple quantitative scoring table is conducive to standardizing the approval process of drugs temporarily procured in cancer hospitals,and ensuring the high-quality,rationality and necessity of drugs temporarily procured.While cooperating with the medical insurance departments to coordinate the drug procurement and the use of management system,the medical institutions should also actively explore the drug management methods to ensure the affordability and multiple selectivity for patients.
