摘要
目的 观察温针法结合毫火针治疗腰椎间盘突出症(lumbar disc herniation, LDH)的疗效及对患者血清白介素6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)水平的影响。方法 选取医院90例LDH患者,根据数字表法随机分为两组,各45例,均予以常规治疗,对照组采用温针法治疗,治疗组采用温针法结合毫火针治疗,比较两组治疗前后中医证候积分、Rdand-Morris问卷(rdand-Morrs questionnaire, RMQ)评分、视觉模拟评分法(visual analogue scale, VAS)评分、血清IL-6及TNF-α水平,并观察两组临床疗效及不良反应情况。结果 治疗组治疗总有效率93.33%(42/45),明显高于对照组77.78%(35/45)(P<0.05);经治疗,治疗组腰痛如刺有定处、肢冷畏寒、拒按、面色■白、膝腿无力及少腹拘急积分明显低于对照组(P<0.05);治疗组RMQ评分、VAS评分均显著低于对照组(P<0.05);治疗组血清IL-6、TNF-α水平明显低于对照组(P<0.05);两组不良反应发生率比较差异无统计学意义(P>0.05)。结论 温针法结合毫火针可更好改善LDH患者症状,降低血清IL-6、TNF-α表达水平,促进腰椎功能恢复,获得更好疗效,且应用安全。
Objective To observe the curative effect of warm needling therapy combined with filiform fire needle therapy on lumbar disc herniation(LDH),and the influence on serum levels of interleukin 6(IL-6) and tumor necrosis factor alpha(TNF-α). Methods Ninety patients with LDH were selected and randomly divided into two groups according to random number table method, with 45 cases in each group. All patients received the conventional treatment. The control group was given warm needle therapy, and the treatment group was given warm needle therapy combined with filiform fire needle therapy. TCM syndrome scores, Rdand-Morris Questionnaire(RMQ) scores, Visual Analogue Scale(VAS) scores, serum levels of IL-6 and TNF-α of the two groups were compared before and after treatment. The clinical effects and adverse reactions in the two groups were observed. Results The total effective rate of treatment in the treatment group was 93.33%(42/45),significantly higher than 77.78%(35/45) in the control group(P<0.05). After treatment, the scores of low back pain with a definite place, cold limbs, refusal to press, pale complexion, weakness of knees and legs and less abdominal arrest in the treatment group were significantly lower than those in the control group(P<0.05). The RMQ scores, VAS scores, serum levels of IL-6 and TNF-α of treatment group were significantly lower than those of the control group(P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05). Conclusion Warm needle therapy combined with filiform fire needle therapy can better improve the symptoms of patients with LDH,reduce the expression levels of serum levels of IL-6 and TNF-α,and promote the recovery of lumbar function, with better curative effect and safety.
