右美托咪定与地塞米松佐剂对腘窝坐骨神经阻滞下足踝手术麻醉及术后镇痛的影响 被引量:4

摘要 目的探讨右美托咪定与地塞米松佐剂对腘窝坐骨神经阻滞下足踝手术麻醉及术后镇痛的影响。方法选择2019年1月至2020年12月收治的100例腘窝坐骨神经阻滞下足踝手术麻醉患者作为研究对象,以简单随机化法将其分为对照组与试验组,各50例。对照组实施超声联合神经刺激器引导下腘窝坐骨神经、股神经阻滞,试验组在对照组基础上联合应用右美托咪定与地塞米松。比较两组的应用效果。结果试验组的感觉阻滞完全起效时间、运动阻滞完全起效时间均早于对照组,阻滞48、72 h镇痛泵用量均少于对照组,镇痛持续时间长于对照组,差异具有统计学意义(P<0.05)。试验组的狭义感觉阻滞持续时间、广义感觉阻滞持续时间均长于对照组,狭义运动阻滞持续时间、广义运动阻滞持续时间均短于对照组,差异具有统计学意义(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论右美托咪定与地塞米松佐剂能够使得罗哌卡因阻滞下腘窝坐骨神经的感觉阻滞与运动阻滞完全起效时间明显缩短,镇痛效果显著,可改善患者预后,值得应用。 Objective To investigate the effects of dexmedetomidine and dexamethasone adjuvant on anesthesia and postoperative analgesia in foot and ankle surgery under popliteal sciatic nerve block.Methods A total of 100 patients underwent foot and ankle surgery under popliteal sciatic nerve block admitted from January 2019 to December 2020 were selected as the study objects,and the patients were divided into control group and experimental group by simple randomization method,with 50 cases in each group.The control group was given ultrasound combined with nerve stimulator guided popliteal sciatic nerve block and femoral nerve block,and the experimental group was given dexmedetomidine and dexamethasone on the basis of the control group.The application effects of the two groups were compared.Results The complete onset time of sensory block and the complete onset time of motor block in the experimental group were earlier than those in the control group,the dosages of analgesia pump at 48 and 72 h after block was less than that in the control group,the duration of analgesia was longer than that of the control group,and the differences were statistically significant(P<0.05).The duration of narrow sensory block and the duration of generalized sensory block of the experimental group were longer than those of the control group,the duration of narrow motor block and the duration of generalized motor block were shorter than those of the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Dexmedetomidine and dexamethasone adjuvant can shorten the complete onset time of sensory block and motor block of popliteal sciatic nerve under ropivacaine block,with significant analgesic effect,and can improve the prognosis of patients.It is worthy of application.
