摘要
目的分析86672例拟输血患者红细胞不规则抗体分布频率及特点,探究其在临床输血中的应用价值。方法以2019年1月至2021年12月在空军军医大学唐都医院住院的86672例拟输血患者为研究对象,采用微柱凝胶法对其进行不规则抗体筛查,对阳性者进行不规则抗体鉴定,分析抗体类型及频率。结果86672例住院患者中不规则抗体阳性609例,检出率0.70%,其中女性阳性率高于男性(0.84%vs 0.56%,P<0.01);有输血史/妊娠史的患者阳性率明显高于无输血史/妊娠史患者(1.18%vs 0.17%,P<0.01)。609例阳性标本中,鉴定出特异性抗体346例,不同血型系统占比依次为Rh系统65.32%,MNS系统20.52%,Lewis系统8.38%,Kidd系统3.75%,Duffy系统0.87%,Diego系统0.58%,P系统0.58%;其中抗E检出率最高(50.87%),其次为抗M(19.07%)及抗Le a(6.94%)。结论对于反复输血患者,自身抗体或药物性抗体等原因无法明确同种抗体特异性的患者,根据本地区不规则抗体分布频率及特点,优先考虑常见的不规则抗体对应抗原阴性供者,选择配合性红细胞输注,对减少输血反应、提高输血质量尤为重要。
Objective To analyze the distribution frequency and characteristics of red blood cell irregular antibodies in 86672 patients to be transfused,and to explore their application value in clinical blood transfusion.Methods A total of 86672 patients who were hospitalized in Tangdu Hospital of Air Force Medical University from January 2019 to December 2021 were selected as the research objects.The irregular antibodies were screened by microcolumn gel method.The positive patients were identified for irregular antibodies,and the types and frequencies of antibodies were analyzed.Results Among the 86672 hospitalized patients,609 were positive for irregular antibodies,with a detection rate of 0.70%.The positive rate of female was higher than that of male(0.84%vs 0.56%,P<0.01).The positive rate of patients with history of blood transfusion/pregnancy was significantly higher than that of patients without history of blood transfusion/pregnancy(1.18%vs 0.17%,P<0.01).Among the 609 positive specimens,346 were identified with specific antibodies.The proportion of different blood type systems was 65.32%in Rh system,20.52%in MNS system.8.38%in Lewis system,3.75%in Kidd system,0.87%in Duffy system,and 0.58%in both Diego system and P system.The detection rate of anti-E was the highest(50.87%),followed by anti-M(19.07%)and anti-Le^(a)(6.94%).Conclusion For patients with repeated blood transfusions,autoantibodies,drug-induced antibodies or other reasons for which the specificity of alloantibodies cannot be identified,the common irregular antibody corresponding to the antigen negative donors should be given priority according to the frequency and characteristics of the distribution in the region.On this basis,selecting compatible red blood cell is significantly important to reduce the incidence of blood transfusion reaction and improve blood transfusion quality.
