摘要
目的系统评估侧壁开窗式上颌窦底提升(LSFA)中骨窗外覆盖屏障膜对窦内新生组织、窦底提升高度、种植体失败和术后并发症的影响。方法电子检索1995年1月-2022年3月PubMed、Embase、Web of Science、The Cochrane Library、中国知识基础设施工程、万方数据库有关LSFA骨窗外覆盖屏障膜的临床研究,且进行引文索引和期刊手动检索。使用Cochrane协作工具评估随机对照研究的偏倚风险,使用Newcastle-Ottawa Scale评估队列研究的偏倚风险。结果共9篇文献被纳入研究,其中8篇随机对照研究,1篇队列研究。Meta分析结果显示,LSFA中,骨窗外盖膜组较未盖膜组呈现更多的窦内新生骨(P<0.05)和残留植骨材料(P<0.05)、更少的结缔组织、更佳的窦底提升高度(P<0.05)和更低的种植体失败率(P<0.05)。术后并发症中骨窗外未盖膜组观察到更多植骨材料移位丢失(7例)和软组织肿胀(7例)。结论LSFA中,骨窗外覆盖屏障膜在窦内新生骨、结缔组织比例、窦底提升高度、种植体预后和术后并发症中具有一定优势,仍需要更多高质量、长期随访的随机对照研究评估屏障膜影响。
Objective To systematically evaluate the osteotomical window with barrier membrane in terms of newly formed sinus tissue,height of sinus floor augmentation,implant failure,and postoperative complica-tions,in lateral sinus floor augmentation(LSFA).Methods Databases in PubMed,Embase,Web of Science,The Cochrane Library,China National Knowledge Infrastructure and Wanfang Data,from January 1995 to March 2022,were searched to identify clinical trials of LSFA,which compared the osteotomical window with and without barrier membranes.Meanwhile citation indexes and manual search were performed.Cochrane tool in randomized controlled trial and Newcastle-Ottawa Scale in cohort studies were employed to assess the risk of bias.Results A total of 9 papers were included,including 8 randomized controlled trial and 1 co-hort study.The Meta-analysis showed that barrier membrane groups resulted in a significantly higher quantity of reconstructed bone(P<0.05)and residual grafted materials(P<0.05),less connective tissue(P<0.05),a superior height of sinus floor augmentation(P<0.05)and lower implant failure(P<0.05).In terms of postop-erative complications,greater grafted materials shifting/loss(7 cases)and mucosal swelling(7 cases)were re-ported in the uncovered membrane.Conclusion In LSFA,employing barrier membrane over the osteotomi-cal window showed a favorable influence in the quantity of reconstructed bone and connective tissue,height of sinus floor augmentation,implant prognosis and postoperative complications.Further evidence from high-quality randomized controlled trial with a long-term follow-up are needed to reevaluate the performance of barrier membrane.
