摘要
目的探究醒脑开窍针联合绳带捆绑技术在脑卒中后偏瘫治疗中的应用价值。方法选取2016年12月—2020年12月期间芜湖市中医医院收治的102例脑卒中后偏瘫患者作为研究对象,采用随机数字表法分为对照组与治疗组,每组各51例。在常规对症支持治疗基础上,对照组配合绳带捆绑技术开展康复训练,治疗组在对照组基础上联合醒脑开窍针治疗,两组均治疗6周。观察比较两组患者临床疗效、治疗前后运动功能(Brunnstrom分期评分)、患肢肌力(MMT分级评分)、患肢肌张力(Ashworth分级评分)、步行功能(Holden分级评分)及自理能力(BI评分)改善情况。结果(1)临床疗效:治疗后治疗组临床总有效率92.16%(47/51)高于对照组78.0%(39/50),差异有统计学意义(P<0.05)。(2)运动功能:治疗后两组患者Brunnstrom分期评分均较治疗前升高,差异有统计学意义(P<0.05);且治疗组高于对照组,差异有统计学意义(P<0.05)。(3)患肢肌力:治疗后两组患者MMT分级评分均较治疗前升高,差异有统计学意义(P<0.05);且治疗组高于对照组,差异有统计学意义(P<0.05)。(4)患肢肌张力:治疗后两组患者Ashworth分级评分均较治疗前降低,差异有统计学意义(P<0.05);且治疗组低于对照组,差异有统计学意义(P<0.05)。(5)步行功能:治疗后两组患者Holden分级评分均较治疗前升高,差异有统计学意义(P<0.05);且治疗组高于对照组,差异有统计学意义(P<0.05)。(6)自理能力:治疗后两组患者BI评分均较治疗前升高,差异有统计学意义(P<0.05);且治疗组高于对照组,差异有统计学意义(P<0.05)。结论醒脑开窍针联合绳带捆绑技术治疗脑卒中后偏瘫效果显著,具有推广应用价值。
Objective To explore the application value of Xingnao Kaiqiao acupuncture combined with rope binding technique in the treatment of hemiplegia after stroke.Method A total of 102 patients with hemiplegia after stroke admitted to the Wuhu Hospital of Traditional Chinese Medicine from December 2016 to August 2020 were selected as the research objects and randomly divided into a control group and a treatment group,with 51 cases in each group.On the basis of conventional symptomatic treatment,the control group received rehabilitation training with rope binding technology,while the treatment group received Xingnao Kaiqiao acupuncture on the basis of the control group.Both groups were treated for 6 weeks.The clinical efficacy,motor function(Brunnstrom stage scores),limb muscle strength(MMT grade scores),limb muscle tension(Ashworth grade scores),walking function(Holden grade scores),and self-care ability(BI scores)were compared between the two groups before and after treatment.Results In terms of the clinical efficacy,the total effective rate of the treatment group was 92.16%,which was higher than 78.0%of the control group(P<0.05).In terms of the motor function,after treatment,Brunnstrom stage scores of the two groups were increased,and the treatment group was higher than the control group(P<0.05).In terms of the muscle strength of affected limb,after treatment,the MMT grade scores of the two groups were increased,and the treatment group was higher than the control group(P<0.05).In terms of the muscle tension of affected limb,after treatment,the Ashworth grade scores of the two groups were decreased,and the treatment group was lower than the control group(P<0.05).In terms of thewalking function,after treatment,the Holden grade scores of the two groups were increased,and the treatment group were higher than the control group(P<0.05).In terms of theself-care ability,Bi scores of the two groups increased after treatment,and the treatment group was higher thanthe control group(P<0.05).Conclusion Xingnao Kaiqiao acupuncture combined with rope binding technique has significant effect on hemiplegia after stroke,which is worthy of promotion and application.
