摘要
目的:探讨通络消散复荣汤对糖尿病周围血管病变患者下肢血流速度及血液流变学的影响。方法:选取符合纳入标准的72例糖尿病周围血管病变患者,依据随机数字表将患者随机分为对照组与观察组,各36例。对照组予以常规西药治疗,观察组在常规西药治疗的基础上予以通络消散复荣汤治疗,共治疗4周。对比治疗前后的踝肱指数(ABI)、搏动指数(PI)、平均血流速度(Vmean)、阻力指数(RI)、全血黏度高切、全血黏度低切、血浆黏度和中医症状积分(下肢发凉、下肢疼痛、间歇性跛行),并对比临床疗效。结果:观察组的总有效率高于对照组(P<0.05)。治疗前,两组患者ABI、PI、Vmean、RI、全血黏度高切、全血黏度低切和血浆黏度比较,差异无统计学意义(P>0.05);治疗后,两组患者各指标均较治疗前改善(P>0.05),且观察组患者ABI、Vmean高于对照组,而PI、RI、全血黏度高切、全血黏度低切和血浆黏度低于对照组(P<0.01)。治疗前,两组下肢发凉、下肢疼痛、间歇性跛行等中医症状积分经比较,差异无统计学意义(P>0.05);治疗后,两组中医症状积分均较治疗前改善(P<0.05),且观察组的下肢发凉、下肢疼痛、间歇性跛行等中医症状积分低于对照组(P<0.05)。结论:通络消散复荣汤治疗糖尿病周围血管病变具有显著疗效,能有效改善下肢血流速度及血液流变学。
Objective:To explore the effect of Tongluo Xiaosan Furong Decoction on blood flow velocity and hemorheology of lower extremities in patients with diabetic peripheral vascular disease(DPVD).Methods:A total of 72 patients with DPVD were randomly divided into the control group and the observation group,with 36 patients in each group.The control group was treated with conventional western medication;on which basis,the observation group was also treated with Tongluo Xiaosan Furong Decoction.The treatment duration was four weeks.The ankle brachial index(ABI),pulsatility index(PI),mean blood flow velocity(Vmean),resistance index(RI),whole blood viscosity of high shear,whole blood viscosity of low shear,plasma viscosity and TCM symptoms score(lower extremity coldness,lower extremity pain,intermittent claudication)were compared between before and after the treatment in the two groups.The clinical efficacy was also compared between the two groups.Results:The total effective rate of the observation group was higher than that of the control group(P<0.05).There were no statistical differences in ABI,PI,Vmean,RI,whole blood viscosity of high shear,whole blood viscosity of low shear and plasma viscosity between the two groups before the treatment(P>0.05);which were all significantly improved after the treatment in the two groups(P<0.05);ABI and Vmean were higher,whereas PI,RI,whole blood viscosity of high shear,whole blood viscosity of low shear,as well as plasma viscosity were lower in the observation group than those in the control group(P<0.01).There was no statistical differences in TCM symptoms scores between the two groups before the treatment(P>0.05);which were all improved after the treatment in the two groups(P<0.05),and the improvements in the observation group were more significant than those in the control group(P<0.05).Conclusion:Tongluo Xiaosan Furong Decoction has a significant curative effect in the treatment of DPVD,it can effectively improve blood flow velocity and hemorheology of the lower extremities.
