摘要
目的系统评价匹多莫德辅助治疗传染性单核细胞增多症(IM)的有效性。方法于PubMed、EMbase、The Cochrane Library、VIP、CNKI、Wanfang Data数据库中检索匹多莫德辅助治疗IM的随机对照试验,两名研究者独立检索筛选数据库中文献时限至2022年2月,纳入研究的文献使用RevMan5.3进行Meta分析。结果与对照组相比,常规疗法基础上,加用匹多莫德能提高治疗IM的总有效率(OR 5.95,95%CI 3.71~9.55),缩短IM患儿退热时间(MD=-2.01,95%CI-2.28~-1.73),改善咽峡炎(MD=-1.42,95%CI-1.67~-1.18),促使肝脾(MD=-2.21,95%CI-2.74~-1.68)及淋巴结(MD=-2.42,95%CI-2.81~-2.04)由肿大至恢复正常,减少CD_(8)^(+)T细胞(MD=-6.37,95%CI-8.11~-4.62),增加CD_(4)^(+)T细胞(MD=5.35,95%CI 4.48~6.22)及CD_(4)^(+)/CD_(8)^(+)值(MD=0.38,95%CI 0.30~0.47)。结论匹多莫德辅助治疗IM有良好的临床疗效,以上结论仍需更多大规模、高质量研究加以验证。
Objective To systematically evaluate the efficacy of Pidotimod in the adjuvant treatment of infectious mononucleosis(IM).Methods Six databases,including PubMed,Embase,The Cochrane Library,VIP,CNKI and Wanfang Database,were electronically searched to collect randomized controlled trials on Pidotimod adjuvant for treatment of IM from inception until February 2022 with two investigators independently.Meta-analysis was performed using RevMan 5.3 software.Results Compared with the control group,adding pidotimod to conventional treatment improved the total effective rate(OR 5.95,95%CI 3.71~9.55),shortened the time of fever(MD=-2.01,95%CI-2.28~-1.73),improved the symptom of angina(MD=-1.42,95%CI-1.67~-1.18),promoted the swollen liver、spleen(MD=-2.21,95%CI-2.74~-1.68)and lymph nodes(MD=-2.42,95%CI-2.81~-2.04)to shrink.In the treatment group,the number of CD_(8)^(+)T cells decreased(MD=-6.37,95%CI-8.11~-4.62),while the number of CD_(4)^(+)T cells(MD=5.35,95%CI 4.48~6.22)and the ratio of CD_(4)^(+)/CD_(8)^(+)(MD=0.38,95%CI 0.30~0.47)increased.Conclusion Pidotimod shows a good efficacy in the treatment of IM.These conclusions need to be further verified by more large-scale and high-quality studies.
