加味麻杏石甘汤对小儿支原体肺炎抗炎因子及机体免疫和外周血ESR、TLR2、TIPE2表达影响 被引量:14

Effects of Modified Maxing Shigan Decoction(加味麻杏石甘汤)on Antiinflammatory Factors,Immunity and Expression of ESR,TLR2 and TIPE2 in Peripheral Blood of Children with Mycoplasma Pneumonia

摘要 目的观察加味麻杏石甘汤对小儿支原体肺炎抗炎因子及机体免疫和外周血Toll样受体2(TLR2)、肿瘤坏死因子α诱导蛋白8样分子2(TIPE2)、血沉(ESR)的表达影响。方法选择2020年5月—2021年5月于秦皇岛市中医医院治疗的90例支原体肺炎患儿,以随机数字表法分组,45例患儿为研究组,45例患儿为对照组,对照组患儿给予阿奇霉素序贯疗法治疗,研究组采用阿奇霉素序贯联合加味麻杏石甘汤治疗,治疗前、后检测两组患儿白细胞介素-10(IL-10)、白细胞介素-2(IL-2)、白细胞介素-17(IL-17)、人半胱氨酰白三烯(CysLTs)、ESR、TLR2、TIPE2、免疫球蛋白IgG、IgA及T淋巴细胞亚群CD_(3)^(+)、CD_(4)^(+)水平,检测两组患儿用力肺活量(FVC)、1秒用力呼气容积(FEV1)、1 min最大通气量(MVV)、呼气峰值流速(PEF)水平,给予两组临床疗效比较。结果研究组IL-17水平低于对照组(P<0.05),研究组IL-2、IL-10水平高于对照组(P<0.05),研究组CysLTs、TLR2、ESR水平低于对照组(P<0.05),研究组TIPE2水平高于对照组(P<0.05),研究组患儿IgA、IgG、CD_(3)^(+)、CD_(4)^(+)水平高于对照组(P<0.05),研究组患儿FEV1、PEF、FVC、MVV水平高于对照组(P<0.05),研究组总有效率高于对照组(97.78%VS 86.67%)(P<0.05)。结论加味麻杏石甘汤治疗支原体肺炎患儿可提升抗炎因子水平,抑制机体炎症,提升机体免疫功能,降低外周血CysLTs、ESR、TLR2水平,提升TIPE2水平,改善患儿肺功能,提升临床疗效。 Objective To observe the effect of Modified Maxing Shigan Decoction(加味麻杏石甘汤,MXSGD)on anti-inflammatory factors,immunity and the expression of Toll like receptor 2(TLR2),tumor necrosis factor inducible protein 8 like molecule 2(TIPE2),erythrocyte sedimentation rate(ESR)in peripheral blood of children with mycoplasma pneumonia.Methods 90 children with mycoplasma pneumonia who were treated in the hospital from May 2020 to May 2021 were selected and grouped by random number table method,45 children were selected as the research group,45 children were selected as the control group,and the children in the control group were given azithromycin sequence The study group was treated with azithromycin sequential combined with Modified Maxing Shigan Decoction.Before and after treatment,the levels of interleukin-10(IL-10),interleukin-2(IL-2),interleukin-17(IL-17),human cysteinyl leukotrienes(CysLTs),ESR,TLR2,TIPE2,immunoglobulin IgG,IgA,T lymphocyte subsets CD_(3)^(+),CD_(4)^(+)were detected.The levels of forced vital capacity(FVC),forced expiratory volume in 1 second(FEV1),maximal ventilation in 1 min(MVV),and peak expiratory flow(PEF)were compared between the two groups.Results The levels of IL-17 in the study group were lower than those in the control group(P<0.05),the levels of IL-2 and IL-10 in the study group were higher than those in the control group(P<0.05),the levels of CysLTs,TLR2 and ESR in the study group were lower than those in the control group(P<0.05),the level of TIPE2 in study group was higher than that in control group(P<0.05).The level of IgA,IgG,CD_(3)^(+)and CD_(4)^(+)in study group was higher than that in control group(P<0.05).The level of FEV1,PEF,FVC and MVV in study group was higher than that in control group(P<0.05),and the total effective rate of the study group was higher than that of the control group(97.78%VS 86.67%)(P<0.05).Conclusion Modified Maxing Shigan Decoction can improve the level of anti-inflammatory factors,inhibit the body inflammation,enhance the body immune function,lower the level of CysLTs,ESR,TLR2 in peripheral blood,elevate the level of TIPE2,improve the lung function and clinical symptoms of children with mycoplasma pneumonia,to improve clinical outcomes.
