摘要
冷文飞教授从事风湿免疫疾病中医临床20余年,对诊治类风湿关节炎(Rheumatoid Arthritis,RA)有着深入的研究与认识。冷师认为RA的发病以肝、脾、肾三脏不足为本,风寒湿热外袭为标,同时又兼湿、痰、瘀夹杂其间。冷师对RA有着独到的诊疗经验:先别寒热以分阴阳;补养肝肾、顾护脾土以治其本;祛寒、化湿、散风,外达其标;化痰通络、活血祛瘀以畅其气血;善用使药,引经报使,直达病所;强调防治养结合,防微杜渐、既病防变。治法上冷师擅用温、清、消、补四法:温阳散寒、清热利湿、消解痰瘀、补养肝脾肾,并总结出“尪痹四味”的用药经验:“热痹四味”“寒痹四味”“乙癸四味”“脾土四味”“风寒四味”“著痹四味”“钻窜四味”,用药特点简练而不拘泥。文章旨在介绍冷文飞教授对RA的中医辨证施治,总结其临证用药规律。
Professor LENG Wenfei has been engaged in the clinical treatment of rheumatoid immune diseases for more than 20 years,and has in-depth research and understanding of the diagnosis and treatment of rheumatoid arthritis.Professor LENG thinks that the incidence of RA is related to the liver,spleen and kidneys three organs weak,the wind,cold,wet and heat attack as the outside reasons;at the same time,wet,sputum,shoal,interspersed also mixed together.Professor LENG has a unique experience in diagnosis and treatment in RA:first distinguish Han(寒)and Re(热)to distinguish Yin(阴)and Yang(阳).Professor LENG has a good use of Wen(温),Qing(清),Xiao(消),Bu(补)four methods on RA,and sums up the"4 kinds of TCM medicines for Wang Bi(尪痹)""Re Bi(热痹)four medicines""Han Bi(寒痹)four medicines""Yi Gui(乙癸)four medicines""Pi Tu(脾土)four medicines""Feng Han(风寒)four medicines""Zhuo Bi(著痹)four medicines""Zuan Cuan(钻窜)four medicines",the characteristics are simple and not rigid.The purpose of this paper is to introduce Professor LENG Wenfei's experience of treating RA based on TCM syndrome differentiation,and summarize the clinical medication rules.
