一氧化碳中毒患者MRI特点及DKI参数变化对病灶预后预判的价值

Value of magnetic resonance imaging characteristics and diffusion kurtosis imaging parameters of patients with carbon monoxide poisoning in predicting the prognosis of lesions

摘要 目的分析一氧化碳(CO)中毒患者磁共振成像(MRI)特点及扩散峰度成像(DKI)参数变化对病灶预后预判的价值。方法选取2018年3月至2021年3月于汉中市中心医院收治的80例CO中毒患者作为病例组,另选取同期30例体检正常者作为正常组,所有受检者均行MRI检查,比较两组受检者及病例组不同预后患者各感兴趣区(ROI)的DKI参数变化情况,采用受试者工作曲线(ROC)分析DKI参数对CO中毒患者预后的预判价值。结果病例组患者MRI影像学显示,主要受累部位为基底节区(45.00%),表现为T1、T2长信号,DWI高信号;病例组患者不同ROI区的FA值、MD值明显低于正常组,MK值明显高于正常组,差异均有统计学意义(P<0.05);预后不佳组患者不同ROI区的FA值、MD值明显低于预后良好组,MK值明显高于预后良好组,差异均有统计学意义(P<0.05);经ROC分析结果显示,半卵圆中心FA值、MD值、MK值对CO中毒患者预后的预判AUC分别为0.614、0.707、0.745;胼胝体FA值、MD值、MK值AUC分别为0.716、0.680、0.662;苍白球FA值、MD值、MK值AUC分别为0.747、0.703、0.685。结论CO中毒患者MRI特点较明显,主要表现为基底节区受累,T1、T2长信号,DWI高信号,且DKI参数对患者预后有较好的预判价值。 Objective To analyze the value of magnetic resonance imaging(MRI)characteristics and diffusion kurtosis imaging(DKI)parameters of patients with carbon monoxide(CO)poisoning in predicting the prognosis of lesions.Methods A total of 80 patients with CO poisoning admitted to Hanzhong Central Hospital between March 2018and March 2021 were enrolled as case group,while 30 healthy persons with normal physical examination during the same period were enrolled as normal group.All subjects underwent MRI examination.The changes of DKI parameters in each region of interest(ROI)were compared between two groups and among patients with different prognosis in case group.The predictive value of DKI parameters for prognosis of patients with CO poisoning was analyzed by receiver operating characteristic(ROC)curves.Results In case group,MRI results showed that the main involvement site was basal ganglia(45.00%),which showed long signals on T1 and T2,and high DWI signals.The fractional anisotropy(FA)value and mean diffusion(MD)value of different ROI in case group were significantly lower than those in normal group,while mean kurtosis(MK)was significantly greater than that in normal group,with statistically significant differences(P<0.05).The value of FA and MD of different ROI in poor prognosis group were significantly lower than those in good prognosis group,while MK value was significantly greater than that in good prognosis group,with statistically significant difference(P<0.05).The results of ROC curves analysis showed that AUC values of FA,MD,and MK in centrum semiovale for predicting prognosis were 0.614,0.707,and 0.745,respectively.AUC values of FA,MD,and MK in callosum for predicting prognosis were 0.716,0.680,and 0.662,respectively.AUC values of FA,MD,and MK in pallidum for predicting prognosis were 0.747,0.703,and 0.685,respectively.Conclusion There are significant MRI characteristics in patients with CO poisoning,which mainly are characterized by involvement of basal ganglia,long signals on T1and T2,and high DWI signals.DKI parameters have a good predictive value for the prognosis of patients.
