摘要
目的系统评价中医外治法治疗骨关节炎的临床疗效。方法计算机分别检索2016年1月至今中国知网(CNKI)、中国生物医学数据库(CBM)、万方(WanFang)、维普(VIP)等数据库收录的中医外治法治疗骨关节炎的临床随机对照试验(RCT)研究,依据文献筛选流程标准最终纳入20篇RCTs。采用RevMan 5.3软件进行Meta分析。结果中医外治法治疗骨关节炎的总有效率、疼痛VAS评分、WOMAC评分明显高于常规西药治疗(P<0.01);采用中医外治法治疗骨关节炎的中医证候积分及Michel Lequesen评分结果显示,中医外治法优于西药治疗(P<0.05)。亚组分析显示,中药外敷组(熏洗、熏蒸、敷贴、热熨和离子导入等)的疗效优于其他外治疗法(推拿疗法、针刀治疗、中医正脊疗法等)(P<0.01);治疗周期对疼痛VAS评分有一定影响,治疗4周组优于治疗2周组(P<0.01)。结论中医外治法治疗骨关节炎能够有效改善患者疼痛VAS评分、WOMAC评分、中医证候积分及Michel Lequesen评分;中药外敷法(熏洗、熏蒸、敷贴、热熨和离子导入等)改善患者疼痛的效果较为突出;连续治疗4周可以更好得改善患者关节的疼痛、僵硬和关节功能情况。
Objective To systematically evaluate the clinical efficacy of traditional Chinese medicine(TCM)external therapy in the treatment of osteoarthritis.Methods From January 2016 to the present,the computer searched the databases of China National Knowledge Infrastructure(CNKI),China Biomedical Database(CBM),WanFang,VIP and other databases respectively for clinical randomization of TCM external therapy in the treatment of osteoarthritis.For random controlled trials(RCT),20 RCTs were finally included according to the criteria of the literature screening process.Meta-analysis was performed using RevMan5.3 software.Results The total effective rate,visual analog scoring method(VAS)score and Western Ontario and McMaster Universities Arthritis Index(WOMAC)score of TCM external therapy for osteoarthritis were significantly higher than those of conventional western medicine treatment(P<0.01).The scoring results showed that the external treatment of TCM was better than that of western medicine(P<0.05).Subgroup analysis showed that the efficacy of external application of TCM(fumigation,application,hot ironing and iontophoresis,etc.)was better than that of other external treatments(Tuina therapy,acupuncture therapy,traditional Chinese medicine chiropractic therapy,etc.)(P<0.01).The treatment period had a certain effect on the VAS score,and that in the 4-week treatment group was better than that in the 2-week treatment group(P<0.01).Conclusion External treatment of TCM for osteoarthritis can effectively improve the VAS score,WOMAC score,TCM syndrome score and Michel Lequesen score.The effect of relieving the pain of patients is more prominent.Continuous treatment for 4 weeks can better relieve the pain,stiffness and joint function of patients.
