摘要
窦性心律震荡(heart rate turbulence, HRT)作为评价自主神经系统完整性和稳定性的可靠指标,在冠状动脉粥样硬化性心脏病、心力衰竭、心肌病、高血压病等心血管疾病中应用广泛,并拓展到了糖尿病、甲状腺功能异常、自身免疫性疾病等领域。中医药通过温阳、滋阴等方法可协调阴阳关系,一定程度上改善机体的自主神经功能,而HRT可反映其改善程度,并对疾病的辨证分型具有指导意义。但目前HRT发生机制尚未明确,且缺乏中药改善机体HRT的机制探索,今后可进一步加强相关的动物实验、分子细胞生物学试验研究,以更好地服务于临床。此外,单一应用HRT评估机体自主神经功能、判断预后具有局限性,需与其他指标联合应用,以提高其灵敏度和特异性,故今后可进一步构建包括HRT在内的由多种自主神经评价指标、预后指标构成的评估体系,弥补单一指标的不足,提高诊疗的准确性。
Heart rate turbulence(HRT) is a reliable indicator for evaluating the integrity and stability of the autonomic nervous system.It is widely used in coronary atherosclerotic heart disease, heart failure, cardiomyopathy, hypertension, and other cardiovascular diseases, and has been extended to diabetes, thyroid dysfunction, autoimmune diseases, and other fields.Traditional Chinese medicine can coordinate the relationship between Yin and Yang by warming Yang, nourishing Yin and other methods, and improving the body′s autonomic nervous function to a certain extent.And HRT can reflect the degree of improvement, and has guiding significance for the syndrome differentiation of the disease.However, the mechanism of HRT is not yet clear, and there is a lack of exploration of the mechanism of traditional Chinese medicine to improve HRT in the body.In the future, relevant animal experiments and molecular and cell biology experiments can be further strengthened to better serve the clinical practice.In addition, the single application of HRT to evaluate the body′s autonomic nerve function and determine the prognosis has limitations, and it needs to be used in combination with other indicators to improve its sensitivity and specificity.Therefore, in the future, an evaluation system consisting of a variety of autonomic evaluation indicators and prognostic indicators, including HRT,can be further constructed to make up for the insufficiency of a single indicator and improve the accuracy of diagnosis and treatment.
