摘要
目的探讨现代中药治疗颈型颈椎病的临床用药特点及配伍规律。方法通过检索中国知网、维普科技期刊数据库、万方医学数据库,收集自建库至2021年6月1日中药治疗颈型颈椎病的相关文献,筛选符合纳入标准的中药处方,建立中药治疗颈型颈椎病数据库,并将相关数据导入古今医案云平台软件,利用频数分析、聚类分析、关联规则、复杂网络等数据挖掘方法,分析现代中药治疗颈型颈椎病的临床用药规律及组方特点。结果最终筛选出符合标准的文献357篇,共388首方剂,合计183味药。高频用药中排名前9的分别为白芍、葛根、桂枝、当归、川芎、甘草、黄芪、羌活、威灵仙,药性以性温占多数,药味则以味辛居多,归经多以入肝、脾、肺经为主;通过系统聚类分析推导治疗颈型颈椎病的新处方4个;由关联规则分析出葛根-白芍,葛根-桂枝,桂枝-白芍,当归-川芎等22个常用药对组合;通过复杂网络获得1个核心处方。结论现代中药治疗颈型颈椎病遵循了祛风散寒、舒筋活血,通络止痛的治疗原则,注重行气、舒筋、活血、祛风,尤其重视解表药、补虚药以及活血化瘀药的应用,用药多以归经肝、脾、肺经,味辛性温的药物为主。
Objective:To explore the clinical medication characteristics and compatibility rules of modern Chinese medicine in the treatment of cervical spondylosis.Methods:By searching CNKI,VIP scientific journal database,and Wanfang medical database,we collected relevant literature on Chinese medicine treatment of cervical spondylosis from the establishment of the database to June 1,2021,screened the Chinese medicine prescriptions that met the inclusion criteria,and established a cervical spondylosis database of Chinese medicine treatment and then imported the relevant data into the ancient and modern medical record cloud platform software.The data mining methods such as frequency analysis,cluster analysis,association rules and complex networks were used to analyze the clinical medication rules and prescription characteristics of modern Chinese medicine for the treatment of cervical spondylosis.Results:357 literatures that met the criteria were eventually screened out,with a total of 388 prescriptions and a total of 183 herbs.The top 9 high-frequency medicines were paeoniae,puerariae,cassia twig,angelica,ligusticun walichii,licorice,astragahis membrancus,notopteryqium root and radix clematidis.Most of the medicinal properties were warm in nature,and most of the medicinal flavors were spicy in taste,mainly entering into the liver,spleen and lung meridians.4 new prescriptions for the treatment of cervical spondylosis were deduced through systematic cluster analysis.A combination of 22 commonly used herb pairs such as puerariae-paeoniae,puerariae-cassia twig,cassia twig-paeoniae,angelica-ligusticun walichii,and so on were obtained through the association rule analysis.Conclusion:Modern traditional Chinese medicine in the treatment of cervical spondylosis follows the principles of expelling wind to dispel cold,relaxing tendons to activate blood,and activating meridian to stop pain,mainly stressing on promoting the circulation of qi,relaxing tendons,activating blood and expelling wind,especially on the application of the medicines such as relieving exterior syndromes,tonifying deficiency and promoting blood circulation to remove blood stasis.The medication is mostly based on the liver,spleen,and lung meridians,which are spicy in taste and warm in nature.
