经鼻高流量氧疗辅助床旁经支气管镜肺泡灌洗的应用研究 被引量:2

摘要 目的 探讨经鼻高流量氧疗辅助床旁经支气管镜肺泡灌洗术(BAL)的应用。方法 选择2020年9月至2021年8月BAL患者60例作为对象,随机数字表法分为2组,各30例。对照组采用鼻导管吸氧行床旁BAL,观察组采用经鼻高流量氧疗辅助行床旁BAL,术后对患者效果进行评估,比较2组血气水平、呼吸相关指标、耐受性及满意度。结果 2组治疗后血气水平得到明显改善;观察组手术后血氧分压(PaO_(2))、血氧饱和度(SaO_(2))高于对照组(P<0.05);血氧二氧化碳(PaCO_(2))低于对照组(P<0.05);2组治疗3 d后呼吸机相关指标得到明显改善;观察组治疗1d后肺顺应性高于对照组(P<0.05);观察组治疗3 d后胸廓顺应性及总动态顺应性均高于对照组(P<0.05);观察组治疗后麻醉耐受、检查耐受、治疗耐受性与干预方法、医患沟通、服务态度满意度高于对照组(P<0.05)。结论 经鼻高流量氧疗辅助用于BAL患者中,有助于改善患者血气水平,能提高患者肺呼吸状态,提高治疗耐受性,获得较高的满意度,值得推广应用。 Objective To explore the application of bedside transbronchoscope alveolar lavage(BAL) with nasal high-flow oxygen therapy.Methods Sixty patients with BAL from September 2020 to August 2021 were selected as subjects and were divided into control group and observation group(n=30) according to the random number table method.The control group used nasal cannula oxygen to perform bedside BAL,while the observation group used nasal high-flow oxygen therapy to assist with bedside BAL.After the operation,the effects of the two methods were e-valuated,and the blood gas level,breathing-related indicators,tolerance and satisfaction of the two groups were compared.Results The blood gas levels of the two groups were significantly improved after treatment,and PaO_(2),SaO_(2) and PaCO_(2) in the observation group were higher than those in the control group after surgery(P<0.05).The ventilatorrelated indexes of the two groups obtained after 3 days of treatment were significantly improved.The lung compliance of the observation group was higher than that of the control group after 1 day of treatment(P<0.05).The thoracic compliance and total dynamic compliance in the observation group were higher than that in the control group after 3 days of treatment(P <0.05).The tolerance after anesthetized treatment,check tolerance,treatment tolerance,intervention methods,doctor-patient communication,and service attitude satisfaction in the observation group were higher than those of the control group(P<0.05).Conclusion The auxiliary use of nasal high-flow oxygen therapy in BAL patients can help improve the blood gas level,pulmonary breathing state,and treatment tolerance.It can obtain higher satis-faction and is worthy of popularization and application.
