摘要
目的:从玄府理论探讨开玄汤治疗轻度溃疡性结肠炎(UC)患者的临床疗效。方法:选取就诊于河北省沧州中西医结合医院的轻度UC患者共98例,按随机数字表法分成对照组及观察组各49例。对照组给予枯草杆菌二联活菌肠溶胶囊联合柳氨磺此啶肠溶片治疗,观察组在对照组基础上加用开玄汤治疗,比较2组治疗后的临床疗效,中医证候评分,改良Mayo活动指数,肠黏膜屏障功能指标,血清炎性指标即白细胞介素-1β(IL-1B)、基质金属蛋白酶(MMP-1)白介素-13(interleukin-13,IL-13),肠道菌群数量,血清miR-155.miR-195相对表达量。结果:观察组(总有效率83.67%)高于对照组(总有效率67.35%),差异有统计学意义(P<0.05),观察组在改善中医证候评分,改良Mayo活动指数评分,血清D-LA、BT、DAO水平,血清炎性指标,肠道菌群上优于对照组,差异有统计学意义(P<0.05),2组治疗后血清miR-155表达水平较治疗前降低,血清miR-195表达水平较治疗前升高,且观察组优于对照组(P<0.05)。结论:开玄汤依据玄府理论,发挥开通玄府、健脾利湿、升举阳气之功,降低中医证候评分,降低血清miR-155表达水平,升高血清miR-195表达水平,减轻炎症反应,改善肠道菌群,修复受损的肠道屏障,达到保护胃肠黏膜的治疗目的,临床疗效显著。
Objective:To explore the clinical efficacy of Kaixuan Decoction in treating patients with mild ulcer-ative colitis(UC)based on Xuanfu theory.Methods:A total of 98 patients with mild UC who were treated in our hospital were selected and divided into the control group and the observation group,with 49 cases in each group according to the random number table method.The control group was treated with Bacillus subtilis combined live bacteria enteric coated capsules combined with sulfasalazine enteric coated tablets,and the observation group was additionally treated with Kaixuan Decoction based on the control group.Modified Mayo activity index,intestinal mucosal barrier function index,serum inflammatory indicators,namely interleukin 1β(IL-1β),matrix metallopro-teinase(MMP-1),interleukin 13(Interleukin-13,IL-13),intestinal The number of bacteria,the relative expres-sion of serum miR-155 and miR-195.Results:The observation group(total effective rate 83.67%)was higher than the control group(total effective rate 67.35%).The difference was statistically significant(P<0.05).The levels of LA,BT,DAO,serum inflammatory indexes,and intestinal flora were better than those of the control group,and the difference was statistically significant(P<0.05).The expression level of 195 was higher than that before treatment,and the observation group was better than the control group(P<C0.05).Conclusion:Kaixuan Decoction,based on the Xuanfu theory,exerts the power of opening Xuanfu,strengthening the spleen and damp-ness,and raising Yang Qi,reducing the TCM syndrome score,reducing the serum miR-155 expression level and increasing the serum miR-195 expression level to reduce inflammation.It can improve intestinal flora,repair the damaged intestinal barrier,and achieve the therapeutic purpose of protecting the gastrointestinal mucosa.The clinical effect is significant.
