摘要
目的分析全生命周期健康教育在非亲缘异基因造血干细胞移植(URD-HSCT)治疗重型地中海贫血患儿中的应用效果。方法选取2018年4月—2020年4月柳州市工人医院收治的82例重型地中海贫血患儿为研究对象,均实施URD-HSCT治疗,采用双盲随机抽样法分为研究组和对照组,每组41例。对照组给予常规健康教育,研究组给予全生命周期健康教育。两组均在健康教育1周后观察效果,比较学前儿童焦虑量表(SCAS)评分、儿童抑郁量表(CDI)评分、治疗依从性、血红蛋白和铁蛋白水平、家属满意度。结果健康教育后,研究组患儿的SCAS评分为(12.16±1.52)分,显著低于对照组患儿的(28.62±2.62)分,差异有统计学意义(t=34.796,P=0.000)。健康教育后,研究组患儿的CDI评分为(52.16±1.66)分,显著低于对照组患儿的(95.26±3.64)分,差异有统计学意义(t=68.983,P=0.000)。研究组总依从率(95.12%)显著高于对照组(73.17%),差异有统计学意义(χ^(2)=7.405,P=0.007)。健康教育后,研究组患儿的血红蛋白和铁蛋白水平分别为(105.26±5.62)g/L和(92.62±5.62)ng/L,显著高于对照组患儿的(92.62±4.74)g/L和(68.26±4.17)ng/L,差异均有统计学意义(t=11.009,P=0.000;t=22.289,P=0.000)。结论地中海贫血患儿在URD-HSCT治疗期间给予全生命周期健康教育可有效减轻患儿焦虑、抑郁等不良情绪,提高治疗依从性、血红蛋白水平、铁蛋白水平及家属满意度,临床应用价值较高,值得进一步推广。
Objective To analyze the application effect of whole life cycle health education in unrelated donor allogeneic hematopoietic stem cell transplantation(URD-HSCT)in treatment of children with severe thalassemia.Methods From April 2018 to April 2020,eighty-two children with severe thalassemia who were treated in Liuzhou Worker’s Hospital were selected as study object,URD-HSCT was performed among these children,all the children were divided into study group and control group according to double-blind random sampling method,forty-one children in each group.The children in control group were given routine health education,and the children in study group were given whole life cycle health education.The effects after one week were observed in the two groups,the scores of Spence Children’s Anxiety Scale(SCAS)and Children’s Depression Inventory(CDI),treatment compliance,the levels of hemoglobin and ferritin,satisfaction rates of family members in the two groups were compared.Results After health education,SCAS score in study group was(12.16±1.52),which was statistically significantly lower than that in control group(28.62±2.62)(t=34.796,P=0.000).After health education,CDI score in study group was(52.16±1.66),which was statistically significantly lower than that in control group(95.26±3.64)(t=68.983,P=0.000).The total treatment compliance rate in study group was 95.12%,which was statistically significantly higher than that in control group(73.17%)(χ^(2)=7.405,P=0.007).The total satisfaction rate in study group was 95.12%,which was statistically significantly higher than that in control group(78.05%)(χ~2=5.145,P=0.023).After health education,the levels of hemoglobin and ferritin in study group were(105.26±5.62)g/L and(92.62±5.62)ng/L,respectively,which were statistically significantly higher than those in control group[(92.62±4.74)g/L and(68.26±4.17)ng/L](t=11.009,P=0.000;t=22.289,P=0.000).Conclusion Life cycle health education in URD-HSCT in treatment of children with thalassemia can effectively relieve anxiety and depression,improve treatment compliance,the levels of hemoglobin and ferritin,and satisfaction rate,the clinical application value is relatively high,the method is worthy of further promotion.
