口腔颌面部恶性肿瘤术后感染相关因素分析 被引量:8

Analysis for correlation factors of post-operation infection in oral and maxillofacial malignant patients.

摘要 目的:分析口腔颌面部恶性肿瘤患者术后颌面部感染病原菌分布及特点。方法:回顾性分析2007年1月~2018年1月新疆医科大学第一附属医院颌面外科收治312例口腔颌面部恶性肿瘤患者病例资料,分析术后感染风险以及病原菌分布。运用Logistic回归分析恶性肿瘤术后感染的相关危险因素。结果:312例恶性肿瘤患者,术后发生感染39例(12.50%)。39例患者细菌培养其中铜绿假单胞菌10例(25.6%),咽链球菌和鲍曼链球菌5例(12.8%),绿脓链球菌3例(7.7%),溶血性链球菌8例(20.5%),金黄色葡萄球菌和肺炎链球菌13例(33.4%)。单因素分析:年龄分布、肿瘤大小和位置、BMI指数糖尿病、手术时间、手术方式、ASA评分、CCI指数、是否输血、术前放疗与口腔颌面部恶性肿瘤术后感染相关。多因素分析:BMI指数、糖尿病、手术时间、ASA指数和手术方式是口腔颌面部恶性肿瘤术后感染的独立危险因素。结论:BMI指数、糖尿病、手术时间、ASA指数、和手术方式是口腔颌面部恶性肿瘤术后感染患者的独立危险因素,故术前需全面评估术后感染风险并给予相关处理。 Objective:To analyzed the distribution of pathogens in those infected patients along with risk factors,in order to provide evidences for treatment and prevention of oral and maxillofacial malignant tumors.Methods:A total of 312 patients with oral and maxillofacial malignant tumors were recruited and investigated for post-op infection rate,distribution of pathogens.Epidemic and tumor factors were included in one-way and multi-factorial Logistic step-regression analysis to reveal related risk factors for post-op infection.Results:Thirty-nine cases(12.50%)of the 312 patients developed postoperative infections.Among them,10 cases(25.6%)were Pseudomonas aeruginosa,5 cases(12.8%)were Streptococcus pharynx and Streptococcus bauman,3 cases(7.7%)were Streptococcus aeruginosa,8 cases(20.5%)were Hemolytic streptococcus,13 cases(33.4%)were Staphylococcus aureus and Streptococcus pneumoniae.Univariate analysis showed that age,size and site of tumour,body-mass index(BMI),diabetes and duration of operation were risk factors.Multivariate logistic analyses,however,showed that BMI,the presence of diabetes,duration of operation and ASA score were the main independent risk factors for postoperative infection.Conclusion:BMI,diabetes,surgery duration,ASA score,and surgery approach as independent risk factors for post-op infection in oral cancers,and pre-op evaluation and related therapy should be performed for post-op infection patients.
