摘要
目的探讨雷公藤多苷片联合百令胶囊治疗Ⅳ期糖尿病肾病(DN)的临床效果。方法选取2017年1月—2021年6月我院120例Ⅳ期DN,按治疗方法分为雷公藤联合百令组、百令组及对照组各40例。对照组予缬沙坦治疗,百令组在对照组基础上予百令胶囊治疗,雷公藤联合百令组在百令组基础上予雷公藤多苷片治疗,均连续治疗6个月。观察3组临床效果,比较治疗前、治疗3和6个月24 h尿蛋白定量和尿足细胞数,以及不良反应发生情况。结果雷公藤联合百令组总有效率高于百令组和对照组,且百令组高于对照组(P<0.05)。治疗3个月时,3组24 h尿蛋白定量、尿足细胞数较治疗前下降或减少,且雷公藤联合百令组低于或少于百令组、对照组(P<0.05)。治疗6个月时,3组24 h尿蛋白定量、尿足细胞数低于或少于治疗3个月时,且雷公藤联合百令组和百令组低于或少于对照组,雷公藤联合百令组低于或少于百令组(P<0.05)。3组治疗后总不良反应发生率比较差异无统计学意义(P>0.05)。结论雷公藤多苷片联合百令胶囊治疗Ⅳ期DN效果确切,可明显降低24 h尿蛋白定量,减少尿足细胞数,保护肾功能,且安全性较高。
Objective To investigate clinical efficacy of Glucoside Tripterygium Total combined with Bailing Capsule in treatment of patients with stageⅣdiabetic nephropathy(DN).Methods A total of 120 patients with stageⅣDN between January 2017 and June 2021 were selected and divided into Glucoside Tripterygium Total combined with Bailing group,Bailing group and control group according to different therapeutic methods.Control group was treated with Valsartan,and Bailing group was treated with Bailing Capsule on the basis of treatment for control group,while Glucoside Tripterygium Total combined with Bailing group was treated with Glucoside Tripterygium Total on the basis of treatment for Bailing group,and all groups were treated for consecutive 6 months.Clinical efficacies were observed,and values of 24-h urinary protein quantitation and numbers of urinary podocytes before and after treatment for 3 and 6 months,and incidence rates of adverse reactions were compared among three groups.Results The total effective rate in Glucoside Tripterygium Total combined with Bailing group was significantly higher than those in Bailing and control groups,and the rate in Bailing group was significantly higher than that in control group(P<0.05).After treatment for 3 months,values of 24-h urinary protein quantitation and numbers of urinary podocytes were significantly decreased compared with those before treatment among three groups,and the value and number in Glucoside Tripterygium Total combined with Bailing group was significantly lower than those in Bailing and control groups(P<0.05).After treatment for 6 months,values of 24-h urinary protein quantitation and numbers of urinary podocytes were significantly lower than those after treatment for 3 months among three groups,and the values and numbers in Glucoside Tripterygium Total combined with Bailing group and Bailing group were significantly lower than those in control group,and the value and number in Glucoside Tripterygium Total combined with Bailing group were significantly lower than those in Bailing group(P<0.05).There were no significant differences in the total incidence rate of adverse reactions among three groups(P>0.05).Conclusion Glucoside Tripterygium Total combined with Bailing Capsule is effective in treatment of patients with stageⅣDN.It may significantly reduce the 24-h urinary protein quantitation and number of urinary podocytes,and protect renal function with good safety.
