摘要
目的分析多排CT在急诊腹部闭合性损伤所致肠系膜损伤诊断中的价值。方法研究对象为佛山市高明区人民医院2017年5月-2020年5月接收的腹部闭合性损伤所致肠系膜损伤患者,共计100例,所有患者均安排应用64排螺旋CT机进行检查,以手术结果作为“金标准”,观察100例患者的CT征象,比较其和手术结果的符合率。结果100例患者多排CT诊断结果显示共计96例患者为腹部闭合性损伤所致肠系膜损伤,4例患者漏诊,诊断准确率漏诊率分别为96.00%和4.00%。100例患者的腹腔积液CT征象和手术对照的符合率为95.45%,腹腔游离气体或肠外积气CT征象和手术对照的符合率为91.18%,肠系膜束带征CT征象和手术对照的符合率为98.00%,肠系膜血管中断CT征象和手术对照的符合率为100.00%,肠系膜脂肪密度升高CT征象和手术对照的符合率为93.85%,肠系膜水中或肠壁间水肿CT征象和手术对照的符合率为90.24%。100例患者对多排CT检查总满意率97.00%(97/100)。结论对于腹部闭合性损伤所致肠系膜损伤患者,多排CT在其急诊诊断中具有良好诊断价值,诊断准确性及满意度均较高,但是其还具有一定漏诊率,因此临床诊断中需要充分结合患者临床症状,密切观察,以进一步减少漏诊情况的发生。
Objective To analyze the value of multi-slice CT in the diagnosis of mesenteric injury caused by closed abdominal injury in emergency.Methods The subjects of the study were patients with mesenteric injury caused by closed abdominal injuries who were treated in our hospital from May 2017 to May 2020.There were 100 cases in total.All patients were arranged to be examined with a 64-slice spiral CT machine,to observe the CT signs of 100 patients and compare the coincidence rate with the surgical results.Results The results of multi-slice CT diagnosis of 100 patients showed that a total of 96 patients had mesenteric injury caused by closed abdominal injury,and 4 patients were missed.The diagnostic accuracy rate of missed diagnosis was 96.00%and 4.00%,respectively.The coincidence rate of the CT signs of abdominal effusion and the surgical control in 100 patients was 95.45%,the coincidence rate of the CT signs of free gas or extraintestinal gas in the abdominal cavity and the surgical control was 91.18%,and the CT signs of the mesenteric banding sign were consistent with the surgical control.The coincidence rate of CT signs of mesenteric vascular interruption and surgical control was 100.00%,the coincidence rate of CT signs of mesenteric fat density and surgical control was 93.85%,and the coincidence rate of CT signs of mesenteric water or intestinal intermural edema and surgical control was 93.85%.The coincidence rate was 90.24%.The overall satisfaction rate of 100 patients with multi-slice CT examination was 97.00%(97/100).Conclusion For patients with mesenteric injury caused by closed abdominal injury,multi-slice CT has good diagnostic value in emergency diagnosis,with high diagnostic accuracy and satisfaction,but it also has a certain missed diagnosis rate,so clinical diagnosis needs to be fully integrated with patients.Clinical symptoms should be closely observed to further reduce the occurrence of missed diagnosis.
