摘要
目的:分析D-D、hs-CRP水平及APACHEⅡ评分对急性脑梗死患者预后的评估价值。方法:选择2017年1月-2021年9月北京市昌平区中西医结合医院收治的急性脑梗死患者113例的病例资料进行回顾性分析。依据患者入院28 d的死亡情况,分为生存组71例和死亡组42例。对比两组入院24 h之内的APACHEⅡ评分、D-二聚体(D-D)、超敏C反应蛋白(hs-CRP)水平。分析患者入院24 h之内的APACHEⅡ评分、D-D、hs-CRP水平对患者预后的预测价值。结果:死亡组入院24 h之内的APACHEⅡ评分、D-D、hs-CRP水平均高于生存组(P<0.05)。入院24 h之内的APACHEⅡ评分、D-D、hs-CRP水平最佳截断值分别为16.16分、1.03 mg/L、43.96 mg/L,约登指数分别为43.99%、36.95%、37.64%。结论:对于急性脑梗死患者,D-D、hs-CRP水平及APACHEⅡ评分可以较为准确地对其近期生存情况进行推断,对患者的病情评估及治疗方案制定具有重要的参考价值。
Objective:To analyze the prognostic value of D-D,hs-CRP levels and APACHEⅡscore in patients with acute cerebral infarction.Method:The case data of 113 patients with acute cerebral infarction treated in Changping District Hospital of Integrated Traditional Chinese and Western Medicine of Beijing from January 2017 to September 2021 were analyzed retrospectively.The patients were divided into survival group(71 cases)and death group(42 cases)according to the death of 28 days after admission.The APACHEⅡscore,D-dimer(D-D)and high-sensitivity C-reactive protein(hs-CRP)levels within 24 hours after admission were compared between the two groups.The predictive value of APACHEⅡscore,D-D and hs-CRP levels within 24 hours after admission in the prognosis of patients were analyzed.Result:The APACHEⅡscore,D-D and hs-CRP levels in the death group within 24 hours of admission were higher than those in the survival group(P<0.05).The optimal cut-off values of APACHEⅡscore,D-D,hs-CRP levels within 24 h after admission were 16.16 points,1.03 mg/L,43.96 mg/L,and Yoden indexes were 43.99%,36.95%,37.64%,respectively.Conclusion:For patients with acute cerebral infarction,the levels of D-D,hs-CRP and APACHEⅡscore can accurately infer their short-term survival,and have important reference value for patients’condition evaluation and treatment plan formulation.
