摘要
目的 总结结肠癌致肠梗阻疾病特点,评价电子计算机断层扫描(CT)在结肠癌致肠梗阻中的诊断价值。方法 80例结肠癌致肠梗阻疾病患者,均经病理证实,分别进行X线(X-ray)检查和CT检查。比较两种检查方法的诊断结果和影像学征象检出情况。结果 病理结果显示梗阻部分分别为盲肠15例、直肠20例、乙状结肠40例、乙状结肠交界5例。CT诊断的准确率96.25%高于X线的75.00%,漏诊率1.25%低于X线的15.00%,差异具有统计学意义(P<0.05);两组患者的误诊率比较,差异无统计学意义(P<0.05)。结肠癌致肠梗阻患者经CT检查直接征象检出狭窄段80例、狭窄段与正常肠管交界处变80例、侵犯周围脏器表现80例,间接征象检出结肠梗阻表现80例、盲肠扩张80例、缺血性结肠68例。结肠癌致肠梗阻患者经X线检查直接征象检出狭窄段0例、狭窄段与正常肠管交界处变0例、侵犯周围脏器表现0例,间接征象检出结肠梗阻表现32例、盲肠扩张32例、缺血性结肠16例。结肠癌致肠梗阻患者经CT检查直接征象狭窄段、狭窄段与正常肠管交界处变、侵犯周围脏器表现和间接征象结肠梗阻表现、盲肠扩张、缺血性结肠的检出率均高于X线检查,差异具有统计学意义(P<0.05)。结论 与X线检查比较, CT检查结肠癌致肠梗阻影像学表现明显,具有较高的确诊率。
Objective To summarize the characteristics of intestinal obstruction caused by colon cancer, and evaluate the diagnostic value of computed tomography(CT) in intestinal obstruction caused by colon cancer. Methods 80 patients with intestinal obstruction caused by colon cancer, all of whom were pathologically confirmed, underwent X-ray examination and CT examination, respectively. The diagnostic results and detection of imaging signs were compared between the two examination methods. Results Pathological results showed that the obstructed parts were cecum in 15 cases, rectum in 20 cases, sigmoid colon in 40 cases,and sigmoid colon junction in 5 cases. The accuracy rate 96.25% of CT diagnosis was higher than 75.00% of X-ray, and the missed diagnosis rate 1.25% was lower than 15.00% of X-ray. All the differences were statistically significant(P<0.05). There was no statistically significant difference in the misdiagnosis rate between the two groups(P<0.05). By CT examination, the direct signs of colon cancer causing intestinal obstruction detected 80 cases of stenotic segment, 80 cases of junction of stenotic segment and normal intestine, and 80 cases of invasion of surrounding organs, and indirect signs detected 80 cases of colonic obstruction, 80 cases of cecum dilatation, and 68 cases of ischemic colon. By X-ray examination, the direct signs of colon cancer causing intestinal obstruction detected 0 case of stenotic segment, 0 case of junction of stenotic segment and normal intestine, and 0 case of invasion of surrounding organs, and indirect signs detected 32 cases of colonic obstruction,32 cases of cecum dilatation, and 16 cases of ischemic colon. The detection rates of direct signs of stenotic segment, junction change of stenotic segment and normal intestinal, invasion of surrounding organs manifestation and indirect signs of colonic obstruction manifestation, cecum dilatation and ischemic colon in patients with colon cancer causing intestinal obstruction by CT examination were higher than those by X-ray examination,and the differences were statistically significant(P<0.05). Conclusion Compared with X-ray examination,CT examination of intestinal obstruction caused by colon cancer has obvious imaging findings, and has a higher diagnosis rate.
